Tooth discoloration due to intra coronal bleeding after trauma can cause aesthetic disturbances and appearance. Internal bleaching is a non-invasive method to restore tooth color which is performed after endodontic treatment by placing a strong oxidizing agent in the pulp chamber. Aim of this study was to report the success of internal bleaching procedure to restore discolored teeth due to trauma. A 44-year-old female came to Conservative Dentistry Universitas Airlangga with chief complaint of the upper right insisor tooth looks darker than the adjacent teeth. The patient had fallen 10 years ago from her motorcycle and her tooth had been painful and then the pain disappeared. In intra oral clinical examination, it showed that tooth 11 had discoloured, vitality test of tooth 11 showed no response to electric pulp test. Radiographic view showed that radiolucen in periapical area and no fracture was found. At the first visit, treatment of single visit root canal using rotary instrument and gave dressing calcium hydroxide. A week later on the second visit, internal bleaching with hydrogen peroxide 35% was done. After bleaching, calcium hydroxide was applied to neutralize the oxidizing agent and then re-evaluated for the color. At the third visit, it showed that the tooth colour was obtain as desire from C3 to A3 (Vitapan Classical). The internal bleaching procedure on the traumatized tooth was the best option to restore the function and aesthetics according to the original tooth colour and also provided benefits in terms of saving time, lower price, and maximum results.

ResumoElementos dentários anteriores estão mais suscetíveis a traumatismos, alterando a estética do indivíduo, pois podem levar à necrose pulpar, com consequente alteração da coloração do dente, estando indicado o tratamento endodôntico e procedimentos para a recuperação da estética. O presente trabalho tem como objetivo relatar um caso clínico de reabilitação estética de um elemento escurecido por trauma dental. Paciente A.C.O.M., 24 anos, compareceu à clínica de Odontologia da Universidade Nilton Lins, Manaus-AM, queixando-se da aparência do dente anterior que escureceu após trauma ocorrido há 01 ano. O plano de tratamento para o caso foi a reabilitação estética do elemento 21 com a associação de clareamento interno mediato e externo imediato após tratamento endodôntico. Pode-se concluir que a associação do clareamento interno mediato com o externo imediato foi uma opção de tratamento adequada, pois o elemento 21 passou da cor C4 para B1 da escala Vita, proporcionando uma excelente estética, harmonização do sorriso e devolução da autoestima do paciente. AbstractUpper dental elements are more susceptible to trauma, altering the individual's aesthetic, as they may lead to pulpal necrosis, with consequent alteration of tooth color, and endodontic treatment and procedures for aesthetic recovery are indicated. The present study aims to report a clinical case of aesthetic rehabilitation of an element darkened by dental trauma. Patient A.C.O.M., 24, attended the Dentistry Clinic at Nilton Lins University complaining about the appearance of the anterior tooth that darkened after a trauma that occurred one year ago. The treatment plan for the case was the aesthetic rehabilitation of element 21 with the association of mediate and immediate internal bleaching after endodontic treatment. It can be concluded that the association of internal mediate and external immediate bleaching was an appropriate treatment option, since element 21 changed from color C4 to B1 in Vita scale, providing excellent aesthetics, harmonization of smile and return of patient's selfesteem. Gigi imatur dengan nekrosis pulpa akibat trauma , karies atau pathosispulpa. Dimana pembentukan dentin terganggu dan perkembangan akarberhenti sehingga terjadi perubahan warna gigi. Mineral trioxideaggregate (MTA) adalah bahan alternatif yang dapat digunakan untukapeksifikasi apeks terbuka karena biokompatibilitasnya, nonmutagenisitas,non-neurotoksisitas, kemampuan regeneratif, dan sifatseal yang baik. Pada kasus ini penggunaan MTA sebagai plug apikal,pada gigi yang imature dengan apeks terbuka merupakan keputusanyang tepat. Tujuan dari laporan kasus ini adalah menjelaskanpenggunaan MTA sebagai apical pluq pada gigi insisivus lateralmaksila dengan diagnosis pulpa nekrosis dengan apeks terbuka dandilanjutkan bleaching internal. Seorang pasien pria 43 tahun yangdatang ke RSGM, Universitas Sumatera Utara dengan keluhan utamagigi yang berubah warna . Pemeriksaan radiografi menunjukkan apeksterbuka untuk gigi #12. Setelah cleaning and shapping dilakukan,kalsium hidroksida ditempatkan sebagai medikamen intracanal. MTAdiletakkan kedalam saluran akar dengan ketebalan 3-4 mmmenggunakan hand pluggers. Obturasi dengan gutta-perchatermoplastik. Penggunaan MTA telah banyak direkomendasikan untukkasus apeks terbuka. Karena memiliki apical seal yang baik,biokompatibilitas dan kemampuan regenerasi jaringan periodontal.Setelah di Follow up tidak adanya keluhan baik secara klinis maupunradiografi. Laporan kasus ini menunjukkan MTA sebagai bahanalternatif untuk apical plug pada metode konvensional apeksifikasi.
