Measurement of intra-abdominal pressure in intensive care units in the United Kingdom: a national postal questionnaire study † †Presented in part at the Association of Anaesthetists Annual Congress, Cardiff, September, 2004.

Abstract: Despite widespread awareness of IAH and the ACS, many intensive care units never measure the IAP. When it is measured, the intravesical route is used exclusively. No consensus exists on optimal timing of measurement or when decompressive laparotomy should be performed.

“…Among them, intravesical pressure measurement is the generally accepted method in the diagnosis. Two previous studies clearly showed that health practitioners were inadequate in measuring and interpreting IAP in the diagnosis of IAH and ACS (13,14). More practical and easily interpretable methods are essential for detecting IAH before it progresses to ACS.…”

“…Furthermore, pigs have a significantly greater BMI than humans (18.5-25kg/m 2 for healthy humans and compared to up to 127kg/m 2 in some breeds of pigs [29]) and obesity in humans is associated with increased IAP [30]. This means that the IAP for pigs should be considerably greater than the approximate 1kPa average seen in humans [30][31][32][33][34][35], though data is not available. These factors might result in a stiffer linea alba in pigs compared to humans.…”

“…Modelling the stress occurring within the linea alba using Laplace's law for thin-walled cylinders, with abdominal dimensions obtained from the literature [36] and a typical extreme IAP of 20kPa [31][32][33][34][35], the linea alba would theoretically experience stresses as high as 1.3MPa which could correspond to a strain of 4.7%. This is under the assumption that the cylinder in question is isotropic and, as such, these values are mid-range of the anisotropic experimental biaxial data.…”
