Methodology and Indications of H2‐Breath Testing in Gastrointestinal Diseases: the Rome Consensus Conference

Abstract: SUMMARY BackgroundBreath tests represent a valid and non-invasive diagnostic tool in many gastroenterological conditions. The rationale of hydrogen-breath tests is based on the concept that part of the gas produced by colonic bacterial fermentation diffuses into the blood and is excreted by breath, where it can be quantified easily. There are many differences in the methodology, and the tests are increasingly popular.

“…The functional H 2 breath test is considered a reliable, non-invasive diagnostic approach for patients with LM and FM [19], but the method is not standardized yet and pre-analytical poor patient preparation during the alveolar air collection may lead to false-negative test results [12]. Moreover, the acidic microclimate in the colon of patients with carbohydrate malabsorption can also cause decreased bacterial H 2 production [20].…”

“…Furthermore an individual and subjective perception of GI symptoms, which patients associate with carbohydrate malabsorption, must be considered [12, 21]. Visceral hypersensitivity is considered to play an essential role in functional symptoms, but the causal symptom triggers are not completely understood yet [3, 19]. …”

“…OCTT was measured by the lactulose H 2 -breath test according to standard guidelines [32,[35][36][37][38]. During the 10 days before the test, antibiotics, probiotics, or other drugs known to affect gastrointestinal motility or intestinal microbiota were prohibited.…”

“…Factors such as the almond genotype [14,15], the growing region [16,17], the climatic conditions during the growing season [18], the harvest time [19], the storage conditions might influence the chemical J Gastrointestin Liver Dis, March 2018 Vol. 27 No 1: [31][32][33][34][35][36][37][38][39] composition of almonds and their nutraceutical effects. Given the high almond consumption, the Italian production cannot cover the whole domestic demand, and a significant amount of almonds must be imported.…”
