Migraine-like Headache after Transseptal Puncture for Catheter Ablation: A Case Report and Review of the Literature

Abstract: Catheter ablation for atrial fibrillation creates an iatrogenic atrial septal defect by transseptal puncture, which may produce a transient right-to-left shunt. We encountered a 44-year-old man who presented with de novo migraine-like headache after cryoballoon ablation for atrial fibrillation. On reviewing the literature, we found additional cases in which migraine-like headache occurred within one week after the procedure and spontaneously within three months. We should recognize migraine-like headache as a … Show more

“…6 In a case series with 571 patients that had received TSC during catheter ablation, 2 patients presented with visual symptoms in the first month after the procedure. 8 At the time of diagnosis, most patients with retinal migraine were less than 40 years old. However, since no further episodes occurred, the ICHD-3 diagnostic criteria for retinal migraine (requiring at least 2 attacks with monocular visual disturbances) were not met.…”

“…This is consistent with the literature with a reported delay to first migraine attacks after TSC of less than a week in most cases and spontaneous remission within 3 months. 8 At the time of diagnosis, most patients with retinal migraine were less than 40 years old. 6 Vasospasms have been proposed to be a possible contributor or epiphenomena in retinal migraine.…”

“…Of the 53 "nonstructural" cases, 13 fulfilled ICHD criteria for MWA (Figure 2). 65,66,68,72,77,82,83,86,87,92,98 In these cases, aura was assumed to be caused by cardiac myxoma (n = 1), viral meningitis (n = 1), head trauma (n = 1, severity not reported), idiopathic intracranial hypertension (n = 1), neurosyphilis (n = 1), cerebral venous thrombosis (n = 1), stellate ganglion block (n = 1), cardiac ablation (n = 4), and the presence of prosthetic heart valves (n = 2). Aura was associated with headache in all cases and preceded the headache in nine.…”

“…Acute (i.e. predischarge) complications were monitored and analyzed prospectively in accordance with the HRS/EHRA guidelines, published systematic reviews and other studies focused on specific complications of single-shot methods [7][8][9][10][11][12]. Long-term complications were determined on the basis of out-patient visits and/or final telephone conversation.…”
