Novel electrocardiographic dyssynchrony criteria improve patient selection for cardiac resynchronization therapy

Abstract: The intra + interDC + TC predicts clinical response after CRT more accurately than TC alone, due to greater TA in the NICD subgroup.

“…CRT plays an important role in the treatment of chronic congestive heart failure [Separham 2019;Hoosain 2017]. It can change the sequence of myocardial activation via left ventricular pacing or biventricular pacing, thereby, improving the LVEF and hemodynamic parameters [Varma 2019;Vereckei 2018]. Many methods have been developed for placing the pacing leads.…”

“…Their marker consists of the time from onset of the QRS to the intrinsicoid deflection in V 1 , V 5 , aVL, and aVF. 4 We think that it is too early to make such strong statement based on a relatively small (124 patients) single-center study. Moreover, their method depends on subjective manual measurements.…”

“…Clinical parameters, ECG features, imaging features, and biomarkers are currently used to predict CRT response. [4][5][6] Among them, ECG is preferred as it is a more feasible test which reflects the underlying structural abnormality manifested as electrical disturbance. [6][7][8] The ECG features for CRT response majorly focus on whether strict Strauss LBBB criteria is fulfilled, as a strict LBBB morphology on surface ECG is suggested to indicate more serious in electrical disturbance rather than structural disturbance in HF.…”
