Novel Metabolic Subtypes in Pregnant Women and Risk of Early Childhood Obesity in Offspring

Abstract: ImportanceThe in utero metabolic milieu is associated with offspring adiposity. Standard definitions of maternal obesity (according to prepregnancy body mass index [BMI]) and gestational diabetes (GDM) may not be adequate to capture subtle yet important differences in the intrauterine environment that could be involved in programming.ObjectivesTo identify maternal metabolic subgroups during pregnancy and to examine associations of subgroup classification with adiposity traits in their children.Design, Setting,… Show more

“…Another study has used cluster analysis in pregnancy to assess the risk of offspring obesity in a general population of healthy pregnant women ( 34 ). Based on nine laboratory variables, they identified five clusters, among others an insulin resistant–hyperglycemic cluster that had the highest rates of obesity in the offspring.…”

“…While the diagnostic criteria for GDM focus on detecting dysregulation of glucose metabolism, GDM is a disorder of all metabolic fuels, and is increasingly recognized as a heterogeneous condition 4,5 . Several upstream determinants of metabolic health are considered risk factors for the development of GDM, including higher body mass index (BMI) as well as sociocultural factors.…”

“…Recently, differences in perinatal outcomes based on physiologic subtypes of GDM (e.g., insulin-resistant vs. insulin secretion deficient) have been described 11 14 . While the diagnostic criteria for GDM detect dysregulation of glucose metabolism, GDM is increasingly recognized as a heterogeneous condition, which may include sub-phenotypes 6 , 15 . As such, metabolic variations, beyond glycemic measures among women with GDM may modify its impact on maternal and fetal health 16 .…”
