The aim of the study was the quantitative assessment of nucleolar organizer regions (NORs) in the cells of preneoplastic and neoplastic endometrial lesions. Retrospective analysis included tissue material obtained from 87 patients with simple and complex hyperplasia with or without atypia and well-differentiated stage-I endometrial adenocarcinoma. Sections were made from routine paraffin blocks and stained with AgNO₃. The mean number of AgNORs per cell was determined. The results obtained indicate that the evaluation of proliferative activity of cells expressed by AgNOR count can help in the distinction between atypical hyperplasia and well-differentiated adenocarcinoma, and thus can serve as a useful pathological criterion.

Σκοπός: η διερεύνηση της συμβολής δεικτών κυτταρικής διαφοροποίησης και κυτταρικού πολλαπλασιασμού, αφ' ενός μεν στην κατανόηση των μηχανισμών της φυσικής ιστορίας των αλλοιώσεων του ενδομητρίου, αφ' ετέρου δε στη διάκριση των καλοηθών από τις κακοήθεις παθήσεις του ενδομητρίου. Μέθοδος: εφαρμόσαμε κυτταροχημικές και ανοσοκυτταροχημικές μεθόδους για την κατάδειξη των περιοχών οργάνωσης πυρηνίου (AgNORs) και τη διερεύνηση της έκφρασης της ογκοπρωτεΐνης Tag-72 και των διάμεσων ινιδίων βιμεντίνης. Αποτελέσματα: παρατηρήθηκε στατιστικά σημαντική διαφορά στην έκφραση μέσου αριθμού agnors ανά πυρήνα, μέσης επιφάνειας agnors και σχέσης επιφάνειας agnors/πυρήνα στις υποομάδες του πληθυσμού μας (ατροφία, πολύπους, υπερπλασία χωρίς ατυπία, υπερπλασία με ατυπία, καρκίνωμα). Η βιμεντίνη βρέθηκε θετική στο 48% των αδενοκαρκινωμάτων. Η Tag-72 πρωτεΐνη εκφράστηκε στο 70% των αδενοκαρκινωμάτων. Συμπέρασμα: η κυτταρολογική προσέγγιση αλλοιώσεων του ενδομητρίου είναι αξιόπιστη και αναπαραγωγική. Η εφαρμογή δεικτών κυτταρικής διαφοροποίησης και κυτταρικού πολλαπλασιασμού επιτρέπει την κατάδειξη του βαθμού αποδιαφοροποίησης και του δυναμικού κακοήθειας των αδενοκαρκινωμάτων του ενδομητρίου σε κυτταρολογικό υλικό.

Women at risk of uterine cancer include those with one or more of the following characteristics: obesity, nulliparity, late menopause, diabetes mellitus, prolonged unopposed estrogen use, and tamoxifen therapy. Risk is additionally increased by the presence of endometrial hyperplasia. The incorporation of biomarkers into the selection criteria of cohort groups at risk for developing endometrial cancer offers an innovative approach to the clinical design of chemoprevention trials of endometrial adenocarcinoma. Biomarkers that may be useful in cohort selection include nuclear morphometry, specific genetic abnormalities, and markers of Proliferation and differentiation. 0 1995 Wiley-Liss, Inc.
