Optimizing Own Mother’s Milk Supply for NICU Babies

Abstract: All babies, including the ones admitted to NICU, should receive their mother’s milk for their reduced morbidity and mortality in short-term and optimal growth and cognitive development in the long run. NICU babies are at the highest risk of lactational failure and formula feeding due to a variety of reasons. Various barriers for exclusive breastfeeding, the quality indicators of optimal lactational supply, and interventions to improve own mother’s milk supply for NICU babies is discussed in this article.

“…By avoiding or reducing the use of fortifiers, feeding concentrated MOM may ensure that these enzymes are most active in the neonatal gut versus interacting with a fortifier while in a storage container. A recent study indicated that most mothers of preterm infants in NICUs who are physically able to and desire to establish lactation can establish an adequate maternal milk supply [35][36][37]. In a survey of parents whose babies had been in NICUs, the majority associated their baby's feeding fortification in the NICU with feelings of powerlessness, inadequacy, disappointment, and worry [38].…”

“…Owing to the many benefits of breastfeeding to the premature neonates, breastfeeding should be encouraged in the NICU. Breastfeeding in the Neonatal Intensive Care Unit (NICU) can be promoted by among others the following ways ( 33 , 57 , 36 ); Family-centred approach Skin-to-skin contact –kangaroo mother care Early and frequently expressing breast milk from both breasts; and Support from peers Lactation counselling Involving mothers in the care of neonate admitted to the NICU Use of human donor breast milk In the event that the milk from the mother is unavailable, human donor breast milk from milk banks can be used. Because it is pasteurized, human donor breast milk has reduced bioavailability than mother's breast milk and may therefore be used only until the mother's breast milk is available ( 1 , 66 ).…”
