Identification and susceptibility testing of oral candidiasis in advanced cancer patients

Background Patients with advanced cancer are prone to develop different opportunistic oral infection due to anti-cancer treatment or the malignancies themselves. Studies of oral fungal samples show an increased prevalence of non-Candida albicans species in mixed oral infections with Candida albicans. Non-C. albicans and C. albicans are associated with varying degrees of resistance to azoles, which may have implications for treatment. This study aimed to assess the diversity and antifungal susceptibility of Candida species detected in the oral cavity. Methods An observational study with microbiological analysis was conducted. Clinical fungal isolates were collected from patients in a hospice unit in 2014–2016. Isolates were re-grown on chromID® Candida plates in 2020. Single colony of each species was re-cultivated and prepared for biochemical identification with a VITEK2® system and verified by gene sequencing. Etest was performed on RPMI agar, and the antifungals fluconazole, amphotericin B, anidulafungin and nystatin were applied. Results Fifty-six isolates from 45 patients were identified. Seven different Candida species and one Saccharomyces species were detected. The results of biochemical identification were confirmed with sequencing analysis. Thirty-six patients had mono infection, and nine out of 45 patients had 2–3 different species detected. Of C. albicans strains, 39 out of 40 were susceptible to fluconazole. Two non-C. albicans species were resistant to fluconazole, one to amphotericin B and three to anidulafungin. Conclusion C. albicans was the predominant species, with a high susceptibility to antifungal agents. Different Candida species occur in both mono and mixed infections. Identification and susceptibility testing may therefore lead to more effective treatment and may prevent the development of resistance among patients with advanced cancer. Trail registration The study Oral Health in Advanced Cancer was registered at ClinicalTrials.gov (#NCT02067572) in 20/02/2014.

O objetivo deste artigo é caracterizar as implicações clínicas orais e a potencial contribuição da Odontologia para a qualidade de vida de pessoas em cuidados paliativos. Foram pesquisados artigos dos últimos 10 anos, relacionados ao assunto nas bases de dados PUBMED e Scielo com as palavras-chave: “cuidados paliativos”, “cuidado dental” e “saúde bucal” e no inglês “palliative care”, “dental care” ou “oral health”. Foram selecionados 7 artigos a partir da busca sistematizada e com a aplicação dos critérios de inclusão e exclusão. As principais manifestações orais de pacientes em cuidados paliativos são a xerostomia, a cárie dental, a candidose e a mucosite, e a presença do cirurgião dentista parece contribuir para a manutenção da qualidade de vida em níveis aceitáveis, ainda que as pessoas estejam em processo de morte. Percebe-se pelo exposto que a abordagem do cirurgião dentista é necessária no âmbito da promoção de saúde, prevenção de agravos e recuperação de reabilitação de danos, que possibilitam a manutenção da qualidade de vida e que esta atenção ainda é negligenciada, devido a falta de obrigatoriedade do cirurgião dentista nas linhas de cuidado de pacientes terminais. Mais estudos sobre a influência dos cuidados odontológicos na manutenção da qualidade de vida em pacientes sob cuidados paliativos são necessários para fomentar e ratificar a importância do cirurgião dentista na equipe de cuidados integrais em saúde desse público.

Funds collection zebranie funduszy Summary Background. Candida is a strain of fungi that is found on the surface of healthy mucous membranes of the oral cavity. Yet, Candida may also be considered opportunistic microorganisms. The pathogenicity of Candida spp. depends on strain-specific characteristics including, invasiveness, adhesion, and production of biofilm abilities as well as the production of enzymes allowing the colonization of tissue. The aim of the paper was to evaluate the frequency of Candida spp. occurrence in the oral microbiota of healthy adults, and its relation with risk factors and colonization. Material and methods. This study consisted of 72 healthy participants (from the Lubelskie Voivodeship between 19 and 73 years of age). Medical history was collected to determine potential relationships to risk factors affecting oral microbiota (past oral cavity diseases, chronic diseases, and smoking). The participants who were qualified had no oral cavity infections during the study. The collected clinical samples (oral cavity swabs) were disseminated on the chromogenic base for isolation and differentiation of Candida spp. Results. The frequency of Candida spp. occurrence was at the level of 63.8%. C. albicans was the most frequently identified species (56.5%). C. glabrata (45.6%) and C. tropicalis (23.9%) were identified less often. In 21.7% of swabs, two species of Candida were identified and in 2.2% of swabs three species identified. The most common colonization risk factor was associated with smoking (32.6%). Conclusions. Yeasts of Candida species are part of the healthy microflora of the oral cavity in people of different ages and may occur as single species or coexist with other species. The coexistence of chronic diseases, propensity for oral cavity infections and smoking significantly influence the Candida colonization. This may result in future clinical consequences, for example in cases of immunodepression.
