Microdissection testicular sperm extraction (microTESE) is considered the gold standard method for surgical sperm retrieval among patients with non-obstructive azoospermia (NOA). In this review, we will discuss the optimal evaluation of NOA patients and strategies to medically optimize NOA patients prior to microTESE. In addition, we will also discuss technical principles and pearls to maximize the chances of successful sperm retrieval, sperm retrieval rates (SRR) based upon testicular histology, predictors of successful sperm retrieval, gonadal recovery following microTESE, and potential complications.

Ook worden de operatietechniek en de work-up van TESE besproken, en de mogelijke complicaties, langetermijngevolgen en controversiële voorbehandelingen. Анализируются прогностические факторы успешного получения сперматозоидов с по-мощью инвазивных манипуляций на яичках. Рассматриваются вопросы гормональной подготовки пациентов с НОА до оперативного вмешательства. Идентификация дополнительных гормональных факторов, таких как фолликулостимулирующий гормон (ФСГ), ингибин В, в сыворотке крови и се-менной плазме позволяет более точно прогнозировать исход микрохирургического извлечения спер-матозоидов (microТЕSЕ). Высокий уровень ФСГ не является противопоказанием для microТЕSЕ у мужчин с НОА. Ни уровень ФСГ, ни объем яичек не связаны с результативностью операции, более того, наблюдается обратная зависимость. Accepted: 20.03.2017 Modern methods of treating infertility in men with non-obstructive azoospermia (NOA) are considered. The prognostic factors of successful sperm production by invasive manipulation on the testicles are analyzed. The aspects of hormonal preparation of patients with NOA before surgery are considered. Identification of additional hormonal factors such as follicle-stimulating hormone (FSH), inhibin B in serum and seminal plasma allows more accurately predict the outcome of microsurgical testicular sperm extraction (MicroTESE). A high level of FSH is not a contraindication for microTESE in men with NOA. Neither the FSH level nor the testicle volume are associated with the effectiveness of the operation, moreover is observed an inverse interrelation. The advantages of microsurgical technique of performing the operation and its effectiveness are considered. Outcomes of microdissection testicular sperm extraction in men with nonobstructive azoospermia due to maturation arrest

Abstract: Sperm were successfully identified in up to one half of the men with MA after micro-TESE. Among men with MA, late MA seems to be the best predictor of successful sperm retrieval with micro-TESE.
