Pelvic epidermoid cyst: A rare cause of lower urinary tract symptoms

Abstract: Pelvic retroperitoneum is a bizarre location of an epidermoid cyst and obstructive voiding caused by it even being stranger. Voiding symptoms related to such an abnormal location of cyst are extremely rare and literature that taps into such cases is scarce. We are presenting here a case of 26-year-old man who was presented with obstructive lower urinary tract symptoms and chronic constipation. On evaluation, he was found to have well-defined cyst in pelvic retroperitoneum. Complete excision of the cyst was don… Show more

“…Once infected, pelvic cysts are very difficult to manage surgically. Instead of undergoing biopsy, rare cases of malignant degeneration of an inclusion cyst can be identified on imaging as a focus of irregular wall-thickening on the cyst on imaging [3] , [12] , [13] .…”

“…Generally, non-infected cysts are easier to dissect and allow for better preservation of the surrounding structures. In young, low-risk patients, cysts should be excised to achieve definitive tissue diagnosis and prevent future complications including fistulization or malignant degeneration [3] , [12] , [13] . In high-risk patients, small, asymptomatic inclusion cysts with benign imaging characteristics can be observed.…”

“…These cysts are often nonenhanced but may show minimal peripheral contrast enhancement. [ 10 ] Given the narrow ureteral cavity and the nature of small lesions, CT diagnosis effect is inferior to MRI. In this present case, we finally decided to perform the ureteroscopy plus biopsy to clarify the nature of the pathological change for the absence of cytological evidence preoperatively.…”

“…These cysts are typically asymptomatic; however, they may lead to the compression of the adjacent pelvic organs and more rarely present as an abdominal mass, or both conditions may coexist, as in the current case. 1 Such cystic lesions may include epidermoid, dermoid, and enteric cysts with malignant pathologies, such as teratoma, teratocarcinoma, and yolk sac tumor. 2 Axial imaging pelvic computed tomography and magnetic resonance imaging (MRI) are the gold standard for diagnosis.…”
