Pemphigus

Abstract: Pemphigus vulgaris and paraneoplastic pemphigus are two subtypes of pemphigus that involve the oral mucosa. These autoimmune blistering disorders have antibodies targeted against proteins of keratinocyte adhesion, thereby causing acantholysis. Clinical findings include oral erosions and flaccid cutaneous bullae and erosions. In addition to the clinical exam, diagnostic tests including tissue biopsy, direct and indirect immunofluorescence, and enzyme-linked immunosorbent assays (ELISA) help to establish a diagn… Show more

“…The mouth is the first site of involvement of PV in about 50% of affected individuals and without therapy perhaps 95% of patients will develop oral lesions. 36,37 Of concern has been the realisation that bullous disease can arise on a background of systemic malignancy, this group of disorders being termed paraneoplastic pemphigus (PNP; sometimes also termed paraneoplastic autoimmune multi-organ syndrome [PAMS]). About two thirds of the associated cancers are haematological (for example, non-Hodgkin's lymphoma, chronic lymphocytic leukaemia and Castleman's disease) although PNP has been observed in association with sarcomas, thymoma, lung cancer and OSCC.…”

“…Paciento gyvenimo kokybės, gretutinių, papildomų sveikatos sutrikimų prevencija priklauso nuo ankstyvos ir tikslios ligos diagnozės, savalaikio šiuolaikinio priešuždegiminio ir imunosupresinio gydymo bei kontrolės. Ankstyva ligos diagnozė ir šiuolaikinis priešuždegiminis ir imunosupresinis gydymas gerokai sumažino pacientų mirtingumą, kuris dabar dažniausiai susijęs su ilgalaikio imunosupresinio gydymo komplikacijomis [1,3].…”
