Percutaneous treatment of gas-containing lumbar disc herniation

Abstract: ✓ A limited number of cases have been reported in which gas-containing lumbar disc herniation caused compression of nerve roots. The authors describe two patients in whom computerized tomography scanning revealed a large intraspinal gas collection that appeared to be causing nerve root compression and that was successfully evacuated by percutaneous needle aspiration.

“…Gas from vertebral disks may also be forced within intraspinal gaseous cysts or gascontaining herniations which may produce compressive radicular symptoms. Gas may also simply migrate in the epidural space (5,(24)(25). In these gaseous cysts or hernias, gas is generally injected under a certain pressure which is obtained in a stepwise fashion by a "ball-valve effect" or a "valve-pump mechanism" (1, 5).…”

“…In the few reported cases of gas-containing cysts, some doctors performed intraoperative or percutaneous needle aspiration of the gas 14,16) , but the majority recommended surgical removal of the gas-filled cyst 2,5,10,11) . We considered the patient's advanced age and decided to perform CT-guided aspiration as the first line treatment.…”
