Colorectal carcinoma is a rare but potentially fatal disease complicating pregnancy. It occurs most frequently in patients aged 50, although some studies report increasing incidence in women under the age of 40. Diagnosis of colorectal cancer during pregnancy is usually made at an advanced stage due to unspecific symptoms. We will present a case of an acute abdomen during pregnancy due to colorectal carcinoma perforation in a 33-year-old patient in her 26th week of gestation. Because of her abdominal condition, left hemicolectomy with colostomy was performed. Two hours after surgery, the patient gave birth to a male child weighing 910 g with an Apgar score of 2/6. The pathohistological finding indicated adenocarcinoma of the colon in Dukes stage B.

As the incidence of colorectal cancer (CRC) in the younger population increases and women start their childbearing journey at later ages, CRC in pregnancy will become an increasingly relevant and decreasingly rare occurrence. Unfortunately, there is a dearth of data on this subject given its historical rarity. CRC is often diagnosed at a late stage in pregnancy because of the conflation of symptoms of pregnancy with symptoms of CRC. A major branch point for the diagnosis and treatment of CRC in pregnancy is whether it is diagnosed early or late in pregnancy. The workup for cancer is similar for pregnant and nonpregnant populations and includes endoscopy and staging computed tomography. Treatment is dependent on the stage of cancer and term of pregnancy. This review summarizes the current evidence for diagnosis, workup, and treatment of CRC during pregnancy and explores issues of fertility after a diagnosis of CRC.

Die Erstmanifestation eines Morbus Crohn in der Schwangerschaft ist ein seltenes Ereignis. Folgend wird über eine 34-jährige Patientin in der 36. SSW berichtet, die wegen Flankenschmerzen rechts mit der Verdachtsdiagnose eines Harnstaus stationär aufgenommen wurde. Im Verlauf wurde unter dem klinischen Bild einer akuten Appendizitis die Notfallindikation zur chirurgischen Intervention gestellt. Intraoperativ fand sich der ausgeprägte Befund einer Ileitis terminalis im Sinne eines Morbus Crohn, der eine Ileozäkalresektion mit Ileoaszendostomie (Seitzu-Seit) notwendig machte. Am 5. postoperativen Tag kam es unter Maximaltokolyse zum Geburtsbeginn mit nachfolgendem Spontanpartus (Frühgeburt). Der weitere postoperative und postpartale Verlauf gestaltete sich für Mutter und Kind komplikationslos. Es werden klinische Symptome sowie die chirurgische und geburtshilfliche Therapie beschrieben und erörtert. Ebenso werden differenzialdiagnostische Schwierigkeiten in der Schwangerschaft sowie die Überlegungen zur Geburtsplanung diskutiert. In the following we report on a 34-year-old patient in the 36th week of gestation, who was suspected of having acute appendicitis. During the operation we found a severe ileitis terminalis like Crohn's disease, which required an ileocaecal resection. In spite of maximal tocolysis she delivered spontaneously 5 days after the operation (preterm delivery). The subsequent developments were uncomplicated. We discuss the clinical symptoms, surgical and obstetric therapies and we review diagnostic problems in pregnancy.
