Periarterial application of papaverine during laparoscopic donor nephrectomy improves early graft function after kidney transplantation in pigs

Abstract: Papaverine substantially improved early graft function in pigs when applied around the renal artery during LDN. Whether this is applicable to procurement of human kidneys remains to be evaluated.

“…External peri-renal vasculature stimuli during surgery can excite the renal and splanchic nerves, which can result from surgical manipulation including mechanical traction. 8,11 The probable cause of AKI in the surgical treatment of this abdominal teratoma resulted from spasm of the left renal artery, which was most likely due to the stimulation of the autonomic nerves surrounding the renal artery. The regulation of renal blood flow is partly due to renal sympathetic nerves, therefore intensification in nerve activity can result in vasoconstriction that consequently disrupts renal blood flow.…”

“…Importantly, renal hilar irrigation with papaverine followed by peritoneum desufflation and placing the kidney at rest for at least 15 minutes before ureteral clipping and division is routinely performed. 3 Renal artery perfusion is confirmed by palpating a tense renal capsule, rather than directly assessing urine output from the transected ureter.…”

“…2 Also, periarterial application of papaverine during laparoscopic donor nephrectomy has been shown to improve early graft function after kidney transplantation in pigs. 3 The proximal ureter has traditionally been left open after division to confirm brisk diuresis and a well-perfused kidney prior to hilar division. 2 …”
