Background Dysphagia is a prominent symptom after esophagectomy and may cause aspiration pneumonia. Swallowing evaluation after esophagectomy can predict and help control the incidence of postoperative pneumonia. The aim of this study was to clarify whether the change in tongue pressure was associated with any related factor and postoperative dysphagia/pneumonia in patients with esophageal cancer after esophagectomy. Methods Fifty-nine inpatients (41 males and 18 females; 33-77 years old) who underwent esophagectomy participated in this study. Measurement of tongue pressure and the repetitive saliva swallowing test (RSST) was performed before esophagectomy (baseline) and at 2 weeks postoperatively. The general data were collected from patients' medical records, including sex, age, type of cancer, cancer stage, location of cancer, operative approach, history of previous chemotherapy, surgical duration, amount of bleeding during surgery, incidences of postoperative complications, intubation period, period between surgery and initiation of oral alimentation, and intensive care unit (ICU) stay, blood chemical analysis, and lifestyle. Results Tongue pressure decreased significantly after esophagectomy (p = 0.011). The decrease of tongue pressure was significantly associated with length of ICU stay and preoperative tongue pressure on multiple regression analysis (p < 0.05). The decrease of tongue pressure in the RSST<3 or postoperative pneumonia (+) group was significantly greater than in the RSST≥3 (p = 0.003) or pneumonia (-) group (p = 0.021). Conclusions The decrease in tongue pressure was significantly associated with the length of ICU stay, preoperative tongue pressure, and the incidence of dysphagia and pneumonia among inpatient after esophagectomy.

Pós-operatório de paciente oncológico em jejum prolongado: diagnósticos e intervenções de enfermagem

Objetivo: identificar os principais diagnósticos de enfermagem e suas respectivas intervenções em uma população de pacientes oncológicos submetidos a jejum prolongado no pós-operatório. Método: estudo descritivo, documental e transversal. O cenário do estudo foi um hospital federal de ensino no Rio de Janeiro, caracterizado como UNACON. Os dados foram coletados em prontuários referentes aos anos de 2016 a 2018 durante 60 dias. Foram analisados 61 prontuários que atendiam aos critérios de inclusão e 208 complicações foram apresentadas pelos pacientes oncológicos. Resultados: Oito diagnósticos foram definidos, dentre os quais os mais evidentes foram: Nutrição desequilibrada: menor que as necessidades corporais e Risco de glicemia instável. Onze intervenções de enfermagem foram definidas em consonância com a Taxonomia de NANDA-I e buscaram ser de fácil aplicação na prática assistencial de enfermagem. Conclusão: Foi evidenciada uma ampla gama de diagnósticos e intervenções de enfermagem a qual se recomenda sua adoção no processo de enfermagem.

Postoperative patients of intestinal reconnection in Morelia, Mexico, are usually in for 3–5 days of oral fast, which increases protein catabolism in the patient, thus lengthening their hospital stay and increasing the risk of developing metabolic comorbidities. Hypocaloric peripheral parenteral nutrition (HPPN) reduces proteolysis and improves inflammatory markers in these patients. The aim of this case report is to determine whether or not peripheral parenteral nutrition (PPN) improves inflammation, lessening the postoperative risk. A 62-year-old female patient and her cancer diagnosis and intestinal reconnection surgery are discussed. PPN is not commonly used in this type of patients due to the short duration of the fast, although its use is common before surgery. However, postoperative use can be beneficial as well, and given the delicate postoperative state these patients are in, it is worth it (at least in these cases) to give them all the strength and tools available for a better recovery. PPN in the case discussed herein improved the patient’s inflammatory marker levels in a shorter period.
