This work focuses on the development of an approach considering the application of the bow tie methodology for monitoring and reporting the performance status of safety barriers used in collapse prevention of dams that store water for refinery process purposes, as well as for mitigation of the consequences in case of materialization of this scenario. After elaboration of the bow tie diagram in collaboration with technical specialists directly involved in dams' safety, the integrity of barriers is determined based on information collected from each dam at an operational level. Considering the literature review carried out, one of the most important contributions of this research (besides the bow tie diagram itself) consists in the proposition of a periodic reporting process of barriers performance indicators to managers responsible for each barrier in order to properly support risk management, permeating all the relevant technical and hierarchical levels (including communications upwards the board—top management). Its main motivation lies in lessons learned from several past major accidents, such as Macondo, Texas City, and, the Brumadinho catastrophe itself, regarding the need for safety information to “travel upwards” on the companies' hierarchy in order to properly and quickly support the decision‐making process, contributing to the strengthening of safety culture inside the organizations.

The complexity of the wharf components and the harshness of the offshore construction environment increase the safety risk of hazards, which has highlighted the importance and urgency of safety risk management in high-pile wharf constructions. This paper established a visualized digital construction safety risk model for high-pile wharf based on a so-called FAHP method (the combination of fuzzy comprehensive evaluation (FCE) and analytic hierarchy process (AHP) methods). The construction safety risk indicators were constructed as the target layer, the principle layer and the scheme layer, and then the corresponding safety risk assessment algorithm was established. The physical, functional and safety risk assessment parameters of the component in the BIM model were employed to the safety risk assessment algorithm, and the risk assessment level of each sub-process was subsequently classified. The case study indicated that the high-pile wharf construction project included five elements in principle layer and 15 risk indicators in the scheme layer. Moreover, it was demonstrated that the sub-processes with the highest construction risk level were steel pipe pile sinking in wharf construction and steel pipe pile, steel sheath-immersed pile sinking and embedded rock pile construction in approaches to bridge construction with a risk level of III. In this way, the quantitative visualization of the construction safety risk was effectively realized, which facilitates the safety risk management of construction sites and timely warning and response to unexpected safety accidents.

In recent decades, process safety control and process safety risk assessment have become very attractive topics. The way control is carried out, using tools such as HAZOP, FMEA, FTA, and similar is not sufficient to adequately prevent or control accidents with serious consequences in the process industry. The need to analyze the causes of the causes themselves, with special emphasis on the impact of organizational factors has arisen. This research results in the development of an original, reliable, and valid measuring instrument for assessing organizational factors important for risk assessment methodologies in working with pressure equipment. The initial instrument was designed based on the previous research and then checked by statistical analysis, using the Kaiser-Meyerto-Olkin test, exploratory factor, and reliability analysis. The proposed instrument has reduced 71 to 48 dimensions, describing 10 organizational factors important for risk management of pressure equipment. Providing a valid and reliable measurement instrument is essential for a proactive approach, which enables managers employed in the organization to mitigate the risks of pressure equipment operation, and prevent accidents. The proposal of further research is the application of confirmatory factor analysis or/and structural equation modeling on data collected.
