Predictive autoimmunity using autoantibodies: screening for anti-nuclear antibodies

Abstract: Background: Early detection of antinuclear antibodies (ANA) in asymptomatic subjects is useful to predict autoimmune diseases years before diagnosis. ANA have been determined by indirect immunofluorescence (IIF) using human epithelial type 2 (HEp-2) cells, which is considered the gold standard technique. Multiplex technology (BioPlex ANA Screen) has been introduced for ANA evaluation in recent years. Nevertheless, concordance between BioPlex and IIF is low and there is no harmonization between both methods for… Show more

“…Among 110 subjects with negative ANA and positive ENA serology, 21% developed to positive ANA during the 2.5-year follow-up. This conversion rate is in agreement with the numbers reported in another study where it was ~25% for two years and ~76% for three years [13].…”

“…For example, 22.5% of the anticellular antibody–negative SLE patients in our study still had anti‐SSA/Ro 60 antibodies using extractable nuclear antigen testing; 11.3% of our anticellular antibody–negative patients had anti‐dsDNA by chemiluminescence immunoassay. Our observations are consistent with a recent study showing that there is significant lack of agreement between positive results using a conventional multiplex array technology and the IIF on HEp‐2 cells .…”

“…Biomarkers such as autoantibodies and a variety of immune‐related and inflammation‐related molecules can appear years prior to clinical symptoms and/or the diagnosis of SLE and can accrue over time . Therefore, longitudinal studies are needed to evaluate the serologic status of anticellular antibody–negative and isolated CMP–positive patients over time and to evaluate whether the status varies with disease activity, damage accrual, therapeutic interventions, and/or specific substrate assays.…”

“…The most prevalent diagnosis was systemic lupus erythematous (37%) followed by Sjögren syndrome (23%). 20 Depending on the population you are looking at, the frequency numbers of WRDs are startling. For example, 1% to 4% of people who carry the gene for CD will manifest celiac.…”
