Predictive value of pancreatic dose-volume metrics on sarcopenia rate in gastric cancer patients treated with adjuvant chemoradiotherapy

Abstract: Gastric cancer with sarcopenia after adjuvant CRT had poorer survival. Higher dose and larger irradiated volume of pancreas correlated with higher risk of sarcopenia. Appropriated administration of pancreas dose-volume may be conducive to reduce the risk of sarcopenia and improve survival in gastric cancer patients treated with adjuvant CRT.

“… 12 Likewise, the sarcopenia cutoff values of an SMI ≤ 49 cm 2 /m 2 for men and ≤31 cm 2 /m 2 for women yielded optimal results. 26 Other cutoff values, such as an SMI < 52.4 cm 2 /m 2 for men and <38.5 cm 2 /m 2 for women 19 , 27 or <43 cm 2 /m 2 for men with a BMI < 25 kg/m 2 and <53 cm 2 /m 2 for men with a BMI ≥ 25 kg/m 2 or <41 cm 2 /m 2 for women, 12 also showed survival differences between the SO and non‐SO groups.…”

“…The latest preoperative CT images were used for the analysis (Figure 1 ). An image slice corresponding to the center of L3 was selected, and from the image, a portion corresponding to fat and muscle was classified according to Hounsfield units (HU): −29 to +150 HU for skeletal muscle 8 , 19 and −190 to −30 HU for adipose tissue. 20 , 21 …”

“…In addition, SkM loss is also associated with reduced tolerance to anticancer treatments and exercise intolerance (fatigue) [ 11 , 12 , 91 ]. “…This involves irradiation of a significant volume of the small bowel and pancreas, increasing the risks of treatment side effects. A study evaluating postoperative radiation to the gastric bed reported incidences of sarcopenia as 29.4% (45/153), 27.3% (35/128) and 37.0% (37/100) prior to CRT, 6 months and 12 monthsafter treatment respectively [63]. Increased radiation dose to the pancreas saw a two-fold increase in sarcopenia prevalence at 12 months (V46 57%).…”
