Predictors of Patient Response to Antibiotic Therapy for the Chronic Prostatitis/Chronic Pelvic Pain Syndrome: A Prospective Multicenter Clinical Trial

Abstract: Culture, leukocyte and antibody status of prostate specific specimens does not predict antibiotic response in patients with the chronic prostatitis/chronic pelvic pain syndrome. The perceived beneficial effect of antibiotics needs to be evaluated in a randomized placebo controlled trial.

“…Recently, a few randomized, placebo-controlled studies showed significant improvement in 50% of CP patients receiving quinolone treatment. Antibiotic treatment was especially effective for patients who had developed CP within the past 4 weeks and had not yet received antibiotic treatment; of these patients, 75% showed signs of relief [12][13][14]. Similar results were also observed in the current study, in which 271 (72.8%) patients used antibiotics; of these, 215 (79.3%) perceived the treatment as effective or very effective.…”

“…NIH-CPSI responders were defined as patients who had experienced a 25% and/or a 6-point decrease (6-point responders) in the total NIH-CPSI score from the baseline values. 16,17 A 5% level of significance was used for all statistical testing, and the statistical analyses were performed using a commercially available analysis program, SPSS (SPSS Inc., Chicago, IL, USA).…”

“…Antibiotics are largely ineffective in the treatment of CP/ CPPS as the chronic nature of the syndrome is thought not to be completely attributable to an ongoing active or latent bacterial infection (43)(44)(45). Despite this, up to almost 80% of CP/CPPS patients receive antibiotics as treatment at some point during their disease course, more than 7 times that of non-CP/CPPS patients, and many receive multiple rounds of antibiotics despite lack of efficacy (46).…”
