Preoperative prediction of histopathological outcome in basal cell carcinoma: flat surface and multiple small erosions predict superficial basal cell carcinoma in lighter skin types

Abstract: The study shows a high accuracy for an overall diagnosis of BCC and increased accuracy in prediction of sBCC for the period when dermoscopy was applied in all cases. The most discriminating findings for sBCC, based on clinical and dermoscopic features in this fair-skinned population, were a flat surface and multiple small erosions.

“…27 In 2016 a two part study of optional then mandatory dermoscopy use, the overall diagnostic sensitivity of basal cell carcinoma (BCC) was 93.3% (91.9–94.5) and specificity was 91.8% (90.6–93.0). 28 Furthermore, dermoscopy when compared to NEE was shown to help predict BCC histopathology subtypes. 28 In the following sections, we will detail further the specific diagnostic features of KC.…”

“…Dermoscopy has significantly increased the accuracy in diagnosing BCC and several specific criteria have been described so far [6]. Furthermore, dermoscopy has also been proven to be useful in BCC subtype prediction, presurgical margin definition, and posttreatment assessment [7][8][9]. To date, only few reports have focused on dermoscopic features of BCC according to body site; in particular, there is a lack of data on BCC located on the lower limbs [10][11][12].…”

“…The named criteria are in consistency with the lesion being an initial BCC before the development of more common criteria according to the known correlations between dermatoscopic and morphological signs [22-27]. In particular, superficial fine telangiectasia and arborizing vessels with a small diameter have been associated with superficial BCCs [25, 26]. Translucency, also called semitranslucency, that can be seen both clinically and dermatoscopically as near-skin-tone colored, smooth, jellylike appearance has been suggested as a dermatoscopic sign of early BCCs measuring only 3–4 mm in diameter [22, 23].…”
