Primary Cutaneous Eosinophil-Rich Anaplastic Large Cell Lymphoma: Report of an Unusual Case and Literature Review

Abstract: To the best of our knowledge, the occurrence of a cutaneous, eosinophil-rich variant of ALCL has not been previously reported. It is important to alert pathologists to this variant of ALCL so that this possibility may be considered in the early differential diagnosis of inflammatory cutaneous conditions.

“…Both cases have already been published. 16,17 With respect to the clinical type, 16 of the 37 cases of ATL were of the smoldering type, 11 were chronic, 4 were acute, 3 were of the lymphoma type, and 3 were of the PCT type. In 19 cases, a microscopic pattern of MF was present ❚Image 1❚, while there were 16 cases of PTCL-U ❚Image 2❚ and 2 of ALCL, both of which were ALK-1 negative.…”

“…The distinct variants of ALCL in the WHO classification are the common (classic), lymphohistiocytic, small cell, and Hodgkin-like variants (1). In addition, other disease variants such as giant cell-rich, sarcomatoid, "signet ring"-like, eosinophil-rich, and neutrophil-rich types, have been recognized (9)(10)(11)(12)(13)(14). The neutrophil-rich form was first described by Mann et al in 1995 (14).…”

“…Because the tumour cell compartment can be obscured by its intense inflammatory background, the diagnosis of neutrophil‐rich ALCL is challenging, especially in skin, which is a predilection site for non‐neoplastic lymphoproliferative diseases. CALCL with extensive neutrophils or eosinophils is extremely rare, with <10 cases reported according to Mann’s criterion 3–9 . Here, we present clinicopathological, immunophenotypic and molecular studies of nine cases with neutrophil/eosinophil‐rich CALCL to delineate further this rare variant and emphasize its differential diagnosis.…”
