Objective: to recognize the role of nurses to maintain continuity of care for users after hospital discharge. Method: an exploratory, descriptive study with a qualitative approach. The study was applied at hospital discharge originating from the most complex health center in Magallanes, the Hospital Clinico Magallanes, Chile. Data were collected through interviews with nurses, carried out between May and August 2018. To analyze the material, the content analysis technique was used. Results: three categories emerged, which bring together what nurses mention in their discourse: How they see the implementation of hospital nursing services, which facilitate continuity of care in the healthcare network of discharged patients (known as care networks); Who they identify in the nursing service for continuity of care after discharge (care networks), which these nursing professionals make and suggestions for their performance; and How care networks affect users in continuity of care after discharge. Conclusion: the role of nurses is key in patient discharge from hospital. With autonomy and competencies for comprehensive care, professionals facilitate the healthcare transition; manage hospital discharge, convening the institutional nursing network and the emerging networks with primary health care; aspire to develop strategies for an inter-level care network, with systematic participation.

We performed a systematic review of the literature to analyze the effect on readmission rates of multifactorial educational interventions at the moment of discharge from the hospital and telephone follow up for patients with heart failure. The quality of reports was analyzed using the CONSORT verification list and the GRADE guide. Using Rev Manager 5.3, the relative risk heterogeneity (RR) of readmission was assessed using I2 and Q statistics, and those considered as homogeneous were combined into a single RR as the confidence interval. We included four reports in the review and three were meta-analyzed. The RR for readmission was 1.01 (95% confidence interval (CI): 0.73-1.40; p = 0.96) with educational interventions and telephone follow-up among patients who visited an emergency room (ER). When studies that included readmission (without ER visit) were combined, the RR was 0.83 (95% CI:0.58-1.17). When studies with visits to ER and readmissions were combined, the RR was 0.91 (95% CI:0.73-1.14). We conclude that educational interventions did not have a significant effect on the rate of readmissions of patients with heart failure.

El soporte ventilatorio no invasivo es una herramienta que ha demostrado mejorar la sobrevida de pacientes con falla muscular de la bomba respiratoria y el manejo de enfermedades pulmonares crónicas, incluso la ventilación no invasiva nocturna ha servido de puente hacia el trasplante pulmonar. Se presenta el caso de una adolescente de 14 años con enfermedad pulmonar crónica hipoxémica severa y falla ventilatoria secundaria, que requirió ventilación prolongada y traqueostomía en espera de trasplante pulmonar. Luego de reevaluar indemnidad de la vía aérea fue decanulada a soporte ventilatorio no invasivo, con uso alternado de mascarilla nasal nocturna y pieza bucal diurna, permitiendo descanso muscular respiratorio eficiente, y mejoría de flujo de tos con técnicas de apilamiento de aire. Este plan permitió una decanulación segura y realizar soporte continuo ventilatorio no invasivo con un programa de rehabilitación cardiorespiratorio. Generalmente, el soporte ventilatorio no invasivo se utiliza en trastornos primarios de la bomba respiratoria. En este caso, se indicó para enfermedad pulmonar crónica hipoxémica, mostrando claros beneficios con oxigenación adecuada, buen rendimiento cardiovascular con mejor tolerancia al ejercicio y entrenamiento en el escenario de preparación al trasplante pulmonar.
