Protective effect of infliximab on methotrexate-induced liver injury in rats: Unexpected drug interaction

Abstract: Inf may partially prevent mtx-induced hepatic damage in rats. However, the combined usage of mtx and inf increases arginase and CPS-1 enzyme activities and at the same time blocks TNF-α. This combination especially in cancer patients may lead to cancer cell invasion and metastasis.

“…Both techniques proved that the reticuloendothelial cells were damaged by ionizing radiation, which might result in malfunction of the Kupffer cells and or a decrease in their number [28]. Histological findings of MTX-induced hepatotoxicity are including macrovesicular steatosis, hepatocellular necrosis and inflammation, edema, fibrosis, morphological abnormalities and development of cirrhosis [29][30][31]. Chemotherapeutic agents caused drug induced acute hepatitis and hepatomegaly, decreased liver enhancement and widening of periportal space due to edema which are nonspecific imaging findings of acute hepatitis [32].…”

“…3,4 Methotrexate is used to treat many different types of cancer (colorectal, breast, and pancreatic cancers) and inflammatory diseases (rheumatoid arthritis − RA, psoriatic arthritis, systemic lupus erythematosus, and dermatomyositis). [1][2][3][4][5] Nevertheless, its use is associated with side effects such as fibrosis affecting the liver, lung, and other organs. 1,2,5 Acute intense MTX side effects are dose-dependent.…”

“…Ib ameliorates carbon tetrachloride-induced liver injury by decreasing the production of transforming growth factor beta and interleukin 1 beta (IL-1β) and by regulating purine metabolism (8). Ib also decreases liver injury by decreasing the production of IL-1β and TNF-α that typically results from methotrexate-induced hepatotoxicity, and by decreasing the formation of peroxynitrite radicals by preventing the formation of excess nitric oxide (7).…”
