Radiotherapy of Conjunctival Melanoma: Role and Challenges of Brachytherapy, Photon-Beam and Protontherapy

Abstract: Conjunctival melanoma is a rare neoplasia, whose therapeutic management is generally of ophthalmological relevance, through radical surgical resection. The high incidence of local relapses after surgery, has made it necessary to combine various types of adjuvant treatments, which in some cases take on the role of radical treatments. Among these non-surgical treatments, those involving the application of ionizing radiation are becoming particularly important. In this review, we discuss the role of episcleral br… Show more

“…Proton therapy and, more broadly, adrotherapy are a milestone in precision RT history. By exploiting a peculiar dose absorption (Bragg peak), they allow us to raise radiation doses almost up to the target edge, with a steep dose fall-off beyond it [19]. To further enhance its potential, proton therapy could be adopted for unconventional use: an almost homogeneous high dose can be delivered deep into the target through a physical grid, whose submillimetric holes would permit sparing of the overlying healthy brain tissue between micron-sized beams [20].…”

“…And external beam radiation therapy is more commonly applied in the patients at a later stage. The benefits and application difficulties of each radiotherapy techniques have been summarized by Spatola et al ( 37 ). Topical chemotherapy is also used as adjuvant therapy of surface tumors, and mitomycin C and 5-fluorouracil are commonly used chemotherapy drugs.…”

“…All patients underwent proton beam radiotherapy at the INFN LNS Nuclear Physics Laboratory (Istituto Nazionale di Fisica Nucleare INFN-Laboratori Nazionali del Sud, Catania), using a superconducting cyclotron that delivers a 62-MeV proton beam [30]. A total dose of 60 GyRBE (Gray relative biological effectiveness, taking into account a constant RBE of 1.1 over the modulated Bragg peak) was split into four consecutive daily fractions of 15 GyRBE.…”
