Aim:The aim of this in vitro study was to assess the amount of apically extruded debris and irrigant using Reciproc vs self-adjusting file (SAF) and to investigate the effect, if any, of gravitational force via mandibular and maxillary positioning of the teeth. Materials and methods:Forty human mandibular premolars were randomly assigned to four groups of 10 teeth each. The root canals were instrumented according to the manufacturers' instructions using the Reciproc and SAF. Sodium hypochlorite was used as the irrigant. The apically extruded debris was collected in pre-weighted glass vials using the Myers and Montgomery method, and teeth were placed in vials both in downward and upward positions. After drying, the mean weight of debris was assessed with a microbalance and statistically analyzed using two-way ANOVA with Bonferroni correction.Results: All instrumentation techniques and positions caused measurable apical extrusion of debris. A significant difference was observed according to position and instrument used (p < 0.05). The Reciproc extruded significantly more debris than SAF and vial downward position extruded significantly more debris than upward position (p < 0.05). Conclusion:Under the conditions of this study, all systems caused apical debris extrusion, especially in the downward position.Clinical significance: According to results of this study, the SAF was associated with less debris extrusion compared to the Reciproc in both mandibular and maxillary positions.

Abstract Introduction:The root canal irrigation aims to clean and disinfect the root canal system, without causing apical extrusion. Objective: To evaluate the influence of apical foramen diameter, needle penetration depth and needle design, and the irrigating solution flow rate on the cleaning and apical extrusion. Material and method: Roots of human mandibular incisors with single-canal were instrumented and after enlarged with two different root apical sizes (40 and 60 K-files), the root canals were filled with contrast solution thickened with propylene glycol and zinc oxide. Two types of irrigation needles (27-gauge) with different designs (apical or lateral opening) were used, two flow rates corresponding to 15 or 25 mL/min and needle penetration at two depths (3 and 6 mm from the apex). Before and after irrigation, the teeth were radiographed using a digital radiographic system. Pre-irrigation (canals filled with contrast) and post-irrigation (canals with remaining contrast) images were submitted to digital subtraction using the Adobe Photoshop CS4 software and the areas were measured using the Image Tool 3.0 software, obtaining the percentage of cleaning and extrusion. Data obtained were subjected to one-way ANOVA and Tukey's tests with the significance level of 5%. Result: There was no statistical difference between the apical foramen diameter, needle penetration depth and needle design, and the irrigating solution flow rate on the cleaning results (more than 97%) and the level of apical extrusion (less than 0.38 mm 2 ), (p>0,05). Conclusion: All irrigation protocols evaluated provided cleaning of the root canal and low apical extrusion of irrigating solution.

Introduction:The apical portion of the root canal is very complex and challenging during endodontic treatment. Root canal preparation and obturation to the apical constriction may provide the best prognosis. Incomplete debridement, foramen transportation, and inadequate seal in the apical portion are considered to be responsible for treatment failure. The technique "apical patency" is considered as a way for maintaining the apical part the free of the debris by recapitulation, using a small K-file through the area of the apical foramen. This term was firstly proposed by Buchanan. In this technique, the smallest diameter file is set 1 mm longer than working length and recapitulated after each instrument to prevent packing of debris in the apical part. Apical patency has been found to be effective in achieving an apical seal with gutta-percha. Teeth prepared with a step back method and with maintained apical patency may show less leakage when obturated with cold lateral condensation technique. Data regarding the effect of apical patency on the healing of periapical tissue are very scarce, and it has been shown that the patency file has detrimental effect on the healing of periapical tissues in animal studies. However, using patency file in endodontic treatment is controversial and further studies are needed. The purpose of this article is to review the effect of using a patency file on the extrusion of root canal contents, the apical seal, postoperative pain, and healing of periapical tissues. Furthermore, the effect of establishing patency on reaching irrigation solutions to the apical portion of the canal and prognosis of root canal treatment are discussed.

Abstract: There was no NaOCl extrusion in nine specimens in P1, but all of these teeth had irrigant extrusion in P2. The 5.25% NaOCl used as an endodontic irrigant showed great capacity to extrude beyond both intact and small-sized apical foramina of MB root canals of maxillary first molars.
