Renal involvement in von Hippel-Lindau disease

Abstract: Renal involvement in von Hippel-Lindau (VHL) disease has emerged as the most prevalent cause of death in this hereditary disorder. In a group of 43 VHL patients (23 unrelated families) with renal lesions we examined whether severity of renal disease is affected by parental inheritance and VHL subtype (1, without pheochromocytoma; 2, with pheochromocytoma). We also tested whether and how nephron-sparing surgery could be applied. Renal involvement comprised multiple cysts and bilateral and multifocal carcinomas … Show more

“…As lesões renais têm, em geral, crescimento lento (12), e como as metástases de carcinoma só são reportadas com tumores maiores que 4,5 cm (15), recomenda-se que sejam abordadas apenas lesões maiores que 3 cm (1). A enucleação simples é indicada em lesões totalmente císticas (14) e a nefrectomia parcial pode ser feita em tumores solitários de até 5cm ou em casos de doença múltipla pouco extensa (15)…”

“…Hypertension, renal function deterioration, and proteinuria have rarely been observed in vHL patients with renal lesions. Cysts as numerous as in polycystic kidney disease are very seldom too (65). Renal cysts and angiomas develop in approximately two thirds of cases (70) and include benign cysts and renal cell carcinoma.…”

“…The von Hippel-Lindau (vHL) syndrome is transmitted as an autosomal dominant trait with variable penetrance. Its clinical manifestations include cerebellar and retinal hemangioblastomas, pancreatic cysts, carcinoma, renal cell carcinoma, and pheochromocytoma (65). The vHL gene is on chromosome 3 (3p25-26) (66) and functions normally as a tumor suppressor by inhibiting transcription elongation (67).…”
