Role of duodenal mucosal nerve endings in the acid‐induced duodenogastric sensorimotor reflex: Effect of benzocaine in healthy humans

Abstract: These findings support a model in which different neuronal subpopulations are responsible for the motor and sensory limb of the acid-sensitive duodenogastric reflex, making benzocaine an unsuitable drug to treat duodenal hypersensitivity to acid. These data provide more insight in the contribution of duodenal neuronal input to gastric physiology in the fasting state.

“…The findings suggested that duodenal acid infusion could induce dyspeptic symptoms in health, further confirming that increased duodenal acid exposure may be a pathogenic factor in FD . Increased duodenal acid exposure further provoked gastroduodenal reflex that induced gastric relaxation and sensitized the proximal stomach to distension, a finding confirmed in later studies …”

“…Another mechanism is a modified duodenogastric reflux due to duodenal hypersensitivity to acid. This duodenogastric reflux determines gastric relaxation, inhibition of antral motility, and sensitization of the proximal stomach to distension 23 . All these influence the perception, interpretation, and the responses to environmental factors which at their turn can be also involved 10,24,25 .…”

“…41,42 A su vez, se demostró que la administración exógena de ácido en el duodeno disminuye el umbral de sensibilidad frente a la distensión gástrica con balón e inhibe la acomodación gástrica posprandial. 43,44 La infusión intraduodenal de lípidos sensibiliza al estómago a la distensión en pacientes con DF, 45 lo que podría explicar el motivo por el cual las comidas grasas pueden empeorar los síntomas relacionados al llenado gástrico.…”
