Background: Safety during patient hospitalization is the patients' right, and the priority of health professionals. Also, a healthcare free from risks and failures is a goal that should be reached by health professionals and a commitment of professional education. Therefore, changes to the nurses' work environment need to focus on enabling and supporting nurses to provide high-quality and safe care Aim: of the study is to develop environmental and patient safety guidelines for nurses in critical care units. Design: the methodological design was conduct to achieve the aim of the current study. Setting: The study was carried out at critical care units of three university hospitals. Subject: the proposed guidelines was distributed to convenient sample of 31 jury members to test its validity and distributed to convenient sample of 162 participant divided to 88 bedside nurses and 74 resident physicians to test the reliability of the proposed guidelines. Results. The proposed guidelines' items were increased to 144 items, and sub-categorized to 17 dimensions. Conclusions: the validity and the reliability of the proposed guidelines were satisfactory, where it can be used in critical care units. Recommendations: use the developed environmental and patient safety guidelines in different critical care units in the selected hospitals and establish a safety committee to facilitate the application of environmental and patient safety committee.

Objectives: to know the meaning attributed by family members to the health safety of pediatric patients, with attention to the possibilities of their collaboration. Methods: this qualitative study was conducted with eighteen family members of children hospitalized in a pediatric unit, from January to July 2018. Symbolic Interactionism was used as a theoretical framework, and Inductive Content Analysis as method. Results: child hospitalization poses risks to possible incidents and adverse events. Participants and professionals are responsible for patient safety. Thus, their actions focus on error prevention. Therefore, they seek information and observe in a vigil way professional care in classic aspects of safety. They conceive essential and favoring safety the approach centered on children and family members. Final Considerations: family members recognized the chances of errors and care damage, identified themselves as support in minimizing damage and were in partnership with the professional, increasing chances of effecting safety.

Objetivo: identificar estudos sobre erros de medicação intravenosa cometidos pela equipe de enfermagem em Unidade de Terapia Intensiva (UTI) adulto. Métodos: revisão integrativa de estudos publicados entre 2015 a 2021 que respondiam à pergunta de pesquisa: “quais erros ocorrem no processo da medicação intravenosa realizado pela equipe de enfermagem nos pacientes adultos internados em UTI?” em 4 bases de dados: Medical Literature Analysisand Retrieval System Online (MEDLINE/PubMed), Biblioteca Virtual em Saúde (BVS), Scopus e Isi Web of Science. Resultados: 39 publicações selecionadas abordam três temas: erros de medicação; incompatibilidade; e intervenções para a redução dos erros de medicação. Identificou-se prevalência de erros de medicação variada entre 1,93% a 64,38%. As incompatibilidades envolvem infusões de midazolam, insulina, norepinefrina, antibióticos e inibidores da bomba de próton. As intervenções aplicadas foram heterogêneas. Conclusão: Por conta de vários fatores, erros de medicação são frequentes na UTI no momento da administração e prescrição.
