Aim of the study was to know the seroprevalence of TORCH infection in antenatal women with bad obstetric history and to co relate the same with different clinical presentations. IgM antibodies for TORCH infection were analyzed in the serum samples of 172 Bad obstetric history (BOH)cases attending Ante Natal clinic by using ELISA according to manufacturer’s instructions. Seroprevalence for , Rubella, CMV and HSV-2 was found to be 12.79%, 11.62%, 04.65% and 02.32% respectively. Highest seropositivity in spontaneous abortions (16.25%)and early neonatal deaths (8.33%) cases was seen with infection. In cases with history of IUD (09.37%) and congenitally malformed babies (22.22%) maximum seropositivity was found for Rubella. History of still births (25%) was equally associated with and Rubella infection. There is a need of screening programmes during antenatal visits to reduce repeated pregnancy wastages in our country. Early detection of IgM by Enzyme-linked immunosorbent assay is useful for diagnosis of TORCH infection to reduce adverse fetal outcome.

Background: TORCH infections, are a medical abbreviation for a set of perinatal infections that are passed from a pregnant woman to her fetus, they are one of the essential causes of Bad obstetric history (BOH). The best identification of TORCH can be done with serological tests by using rapid test and ELISA. Patients and Methods: A total of 300 women with BOH were enrolled in this study, rapid test and ELISA technique were applied to detect of TORCH infections in study group beside 30 women as the control group. Results: The age group was ranging between 18-42 years, the highest percentage of IgG was specific for HSV-1 and Rubella virus which were 57.9% and 54.6% respectively, the highest percentage of IgM was specific for Rubella virus and Toxoplasma which was 2% for both of which. The positive seroprevalence for TORCH infection was 89% of the study group. The frequency of abortion was the highest between the other BOH classes 57.3%, followed by early neonatal death 14%, Intrauterine death 7% and 6% for congenital anomalies. The results also showed that there were no significant differences between the results of ELISA technique and rapid test to the IgG and IgM for all items of Torch infection Conclusions: The most frequent age was between 25-30, the highest decrease in levels of anti-TORCH IgM and increase the percentage of different anti-TORCH IgG which results in highly increasing in positive seroprevalence for THORC infection. The recent rapid tests are very specific and their result is similar to the results of automated ELISA. TORCH group of pathogens is responsible for serious congenital infections, leading to fetal damage and other anomalies. A national screening program for TORCH infections does not exist in India. Serological data regarding for TORCH infections during pregnancy is not representative for India. Settings and Design: This prospective study was done over a period of one year at a tertiary care center in North India. We enrolled 419 pregnant women with bad obstetric history. Serological evaluation for TORCH infections was carried out by IgM Enzyme Linked Immunosorbant Assay (ELISA) method. Statistical analysis used: Gaussian (z) test. Overall, 260 (62.1%) samples were negative for TORCH pathogens while 159 (37.9%) were positive. The IgM sero positivity to Toxoplasma gondii, Rubella, Cytomegalovirus (CMV) and Herpes simplex virus (HSV-2)was 16.4%, 8.8%,10.2% and 2.3% respectively. Maximum seropositivity was observed between 21-30 year age group. Out of the total positive cases, 6 (3.7%) were found to be coinfected. The maximum numbers of coinfection cases was 5 (83.3%) with Toxoplasma, followed by Rubella with one case (16.6%). TORCH epidemiology needs better understanding for development of new strategies for the prevention of congenital infections. New approaches to prevention and treatment of congenital TORCH infection are necessary, including antiviral interventions and the development of a vaccine strategy.
