Objective: Apelin is a proinflammatory adipocyte-derived factor. The aim of this study was to evaluate the role and significance of serum apelin as a new sepsis marker in the identification of full-term and preterm new-born infants with early-onset sepsis. Material and Methods: This was a case-control study. We included 80 neonates. The cases were divided into 2 groups; neonates with early-onset sepsis and control group with neonates non-sepsis. Apelin was quantified by enzyme-linked immunosorbent assay. Results: There was a significant elevation in the mean values of serum apelin in the early-onset sepsis group (1214.7 ± 273.06 pg/mmol) than in the non-septic neonates 116.27 ± 21.96 pg/mmol ( P < .0001). Apelin values were correlated to clinical sepsis and hematological scores as well as C-reactive protein. Serum apelin concentration was significantly higher among culture-positive cases than the culture-negative cases (mean ± SD was 1239.52 ± 268.47 and 929.42 ± 136.97 pg/mmol, respectively, P < .0001). Moreover, the apelin level was higher in non-survivor neonates than in the survivors in the early-onset sepsis group. No significant difference was found between preterm and full-term new-born infants with regard to the apelin values. The best cut-off estimate of apelin to diagnose early sepsis was >178.33 pg/mmol. Conclusion: Apelin may be useful in the diagnosis and prognostic prediction of neonates with early-onset sepsis.

Background: Infant morbidity as well as mortality are frequently caused by neonatal sepsis. Neonatal sepsis can be predicted using the red cell distribution width (RDW), according to several researches. Objective: To determine if RDW can be employed as a marker for the evaluation of newborn sepsis and the assessment of its severity. Patients and Methods: 40 newborns, 20 of whom were infected and the other 20 of whom were non infected, participated in this case-control research. Patients and controls were collected from neonatal intensive care unit (NICU), Zagazig University Hospitals. Full history was taken from all participants parent, with clinical and laboratory examination were done; complete blood picture, and creatinine, blood culture, and serum level of C-reactive protein (CRP). Results: We revealed significant link between RDW and all of total leukocyte count (TLC), immature to total neutrophil ratio (I/T ratio), absolute neutrophil count (ANC), CRP, procalcitonin, severity of sepsis, and mortality. RDW and platelet count, on the other hand, have a strong negative association. With a sensitivity of 83.3 percent, specificity of 50 percent, a positive predictive value (PPV) of 71.4 percent, and a negative predictive value (NPV) of 66.7 percent, accuracy of 70 percent (p>0.05), the best RDW cutoff for diagnosing newborn sepsis severity was ≥17.9. Conclusion: Predictors of illness severity and death in newborn sepsis may be accurately predicted using baseline RDW measurements, which is critical for treatment of neonates who are at great risk of sepsis.

Biała tkanka tłuszczowa jest hormonalnie aktywnym narządem wydzielania wewnętrznego, zdolnym do produkcji i uwalniania wielu adipokin, w tym apeliny 12, znanej jako ligand receptora sprzężonego z białkiem G -APJ. Układ apelina/APJ występuje w wielu tkankach organizmu, odgrywając ważną rolę w utrzymaniu wrażliwości na insulinę, przyjmowaniu pokarmu i w patomechanizmie otyłości u dzieci. Apelina reguluje rozwój i czynność układu sercowo-naczyniowego, ciśnienie krwi, kurczliwość mięśnia sercowego, angiogenezę, metabolizm węglowodanów i tłuszczów, proliferację komórek, apoptozę i procesy zapalne. Hamuje wchłanianie wody i produkcję wazopresyny. Największe jej stężenie stwierdza się w śródbłonku naczyń, mięśniach gładkich naczyń krwionośnych, w mózgu, sercu, płucach, jądrach, jajnikach, wątrobie, łożysku, gruczole piersiowym i mleku kobiecym. Odgrywa ona kluczową rolę w homeostazie poprzez hamowanie aktywacji płytek krwi. Ekspresja układu apelina/APJ jest znacznie nasilona w atrezji dróg żółciowych, zwłaszcza w schyłkowej marskości wątroby. W przewlekłej niewydolności krążenia osoczowe stężenie apeliny ulega zmniejszeniu, a u pacjentów z nadczynnością tarczycy osiąga wysokie wartości. Apelina jest potencjalnym markerem diagnostycznym w różnicowaniu choroby moyamoya z miażdżycą naczyń mózgu i nowym biomarkerem cukrzycy typu 2 u mężczyzn. Dzieci z pierwotnym nadciśnieniem tętniczym mają wyższe stężenia apeliny niż dzieci zdrowe. Apelina ma znaczenie w patogenezie dziecięcej astmy oskrzelowej. U niemowląt z nadmierną urodzeniową masą ciała obserwuje się znaczną hiperapelinemię. Apelina może działać jako czynnik mitogenny dla osteoblastów, bierze udział w rozwoju nefropatii cukrzycowej, w nowotworzeniu naczyń w retinopatii wcześniaków. Podkreśla się jej wartość diagnostyczną w rozpoznawaniu sepsy u noworodków.
