Objectives The purpose of the study was to assess the occurrence, location, and dimensions of the intraosseous vascular canal in the lateral wall of the maxillary antrum using cone-beam computed tomography (CBCT). Materials and Methods In this retrospective study, we examined 400 CBCT scans from our archive of patients who had earlier reported to a dental teaching hospital in the United Arab Emirates. The prevalence, location, and dimensions of the lateral antral intraosseous canal (LAIC) in the maxillary antrum were evaluated by 2 examiners using standardised methods. A third examiner was consulted in cases of disagreement. Results The prevalence of LAIC was 62.3% (249 maxillary antra) among the study population. The mean distance between the most inferior point of the alveolar bone and the inferior border of the LAIC in the posterior maxillary region was 19.83±3.12 mm. There was a significant difference ( P =0.05) between the maxillary molar and premolar regions in mean distance from the most inferior point of the alveolar bone and the inferior border of the LAIC. There was no statistically significant difference in mean distance between the most inferior point of the alveolar bone and the inferior border of the LAIC between dentulous and edentulous areas ( P =0.1). The G3-intrasinusal type canal less than 1mm in diameter was the most common type of LAIC. Conclusion This study established the approximate location of the LAIC in a United Arab Emirates cohort, which will assist the oral surgeon in selecting the appropriate site for sinus lift procedures with reduced risk of surgical hemorrhage.

De forma reabilitadora e estética, a ciência da implantodontia tem se aprimorado com o intuito de solucionar alguns incômodos relatados por pacientes. Dentre essas queixas, as zonas edêntulas devem ser meticulosamente avaliadas para um adequado planejamento clínico. Assim sendo, quando é diagnosticado o quadro de maxila atrófica, na maioria dos casos o seio maxilar acaba pneumatizando e técnicas operacionais são analisadas para possibilitar a acomodação dos implantes. Por essa razão, o objetivo deste trabalho é de relatar as principais complicações decorrentes do procedimento de levantamento do seio maxilar. Para a elaboração da pesquisa, foi realizado levantamento bibliográfico, utilizando as bases de dados Scientific Electronic Library Online (Scielo) e Medical Literature Analysis and Retrieval System Online (Medline). Descritores específicos como levantamento do assoalho do seio maxilar, complicações intraoperatórias e maxila, foram utilizados como método de busca. Nessa pesquisa, os artigos foram selecionados por critérios adequados relevantes à temática, todos publicados entre o período de 2016 a 2022, nos idiomas inglês e português. Na literatura vigente, muitos são os estudos nessa área que comprovam grande índice de sucesso nas cirurgias de levantamento do seio maxilar, no entanto, assim como qualquer intervenção cirúrgica, riscos operacionais podem ser presentes. Nesse aspecto, quando algumas questões são somadas como bom conhecimento científico sobre o assunto, destreza manual do operador e respeito às conformidades de cada indivíduo, a probabilidade de complicações da cirurgia diminui.

Maxillary sinus lift surgery is applied to compensate for the reduced vertical height in the posterior maxilla to facilitate placing a dental implant of a suitable length. Pathological conditions may be accidentally discovered, which necessitate careful assessment and management to prevent the infection of the maxillofacial complex and eventually bone grafting and dental implant failure. This case report describes an approach for the management of Schneiderian membrane perforation associated with the removal of an antral pseudocyst for successful dental implant therapy. A 70-year-old healthy Caucasian male presented for implant therapy to replace a non-restorable maxillary molar. Initial examination revealed the need for a sinus lift procedure to prepare the site for implant placement. A 3D CBCT evaluation before surgery revealed an incidental finding of a pathological lesion at the surgical site. The histological analysis of a biopsy specimen retrieved during implant site preparation showed findings consistent with antral pseudocyst. The resulting perforation of the sinus membrane was treated, and an adequate period of healing was given. A thickened sinus membrane was detected upon surgical exposure for implant placement. The novel technique illustrated could result in a fibrotic repaired sinus membrane and help shorten the time required for dental implant treatment.
