Spectrum of malignancies in human immunodeficiency virus – positive patients at a Tertiary Care Centre in South India

Abstract: Malignancies in HIV positive individual occurred in younger individuals. Non-Hodgkin lymphomas, especially extra-nodal lymphomas, were the most common malignancy. There were no cases of proven Kaposi's sarcoma or invasive cervical carcinomas. There were two cases of multiple myeloma which are infrequently reported.

“…10 e authors mentioned that this nding might be because of the low seroprevalence of Kaposi sarcoma-associated herpesvirus in the Asian population. 10 ree di erent studies from southern India have also not reported the incidence of Kaposi Sarcoma in their series of HIV-AIDS patients with malignancies, [11][12][13] and similar ndings were also reported in a study from northern India. 9 e incidence of other immunode ciency-related malignancies was identical to those reported in other studies in the literature.…”

“…At the same center as the present study (AIIMS), a retrospective study analyzing malignancy in HIV patients during the initial years of ART (2005-2013) suggested a similar infrequent occurrence of malignancy in HIV, with NHL being the most common [30]. Most regional studies from India concur that NHL is the most common malignancy in HIV and KS is very rare [21-23, 29-33]. One of the reasons postulated is the low seroprevalence of HHV-8 in the Indian population [34].…”

“…In 2016, there were 36.7 million people living with HIV worldwide; 2.1 million of these lived in India [19, 20]. Based on regional records, NHL is the most common cancer overall [21-24] and Kaposi’s sarcoma is rare [23]. Increasing access to ART [25] is expected to decrease the overall incidence of cancer in this population, but the spectrum of cancer in people living with HIV in India needs to be better understood for effective screening.…”

“…The spectrum of HIV/AIDS-related malignancies in India differs from that reported in the Western literature [5910]. This difference may be due to the low prevalence of human herpesvirus 8 in the Indian population or to differential patterns of HIV transmission, with the heterosexual route the most common in India [11].…”

“…The exact burden of HIV/AIDS-associated cancers in India is not known. Kaposi's sarcoma is the most frequent AIDS-defining cancer in the Western world, while non-Hodgkin's lymphoma (NHL) is most commonly reported in Indian literature [456]. We undertook the current study to evaluate the epidemiology and outcomes of HIV/AIDS-related lymphoma (ARL) in a tertiary cancer center in India.…”
