Testicular Rupture or Testicular Fracture? A Case Report and Literature Review

Abstract: Testicular trauma is relatively uncommon. However, severe injuries can result in many complications and should be carefully diagnosed and managed. We present a case of testicular fracture diagnosis made by ultrasonography. The surgical exploration revealed the fracture as well as complete rupture of the tunica albuginea. Testicular rupture is the disruption of the tunica albuginea, while testicular fracture is a “break” in the testicular parenchyma. Management could be conservative in mild fracture cases witho… Show more

“…Testicles are predisposed to traumatic injuries because of their external location. Testicular trauma should be treated as an emergency condition because exposure of the spermatic antigen to the immune system [199] may induce an auto-immune response against sperm through the production of anti-sperm antibodies (ASA) [200]. Among infertile males, 8-21% have ASA that can negatively affect sperm motility, capacitation, and acrosome [201].…”

“…In one report, 28% of testicular injuries from trauma also presented with associated perineal soft tissue injuries. Although in general a rare entity in blunt trauma situations, testicular trauma is often associated with motorcycle collisions (3,4). These are usually assumed to be from contact with the fuel tank, which strikes the groin and results in damage to the scrotum and perineum, commonly called a fuel tank injury (3,5).…”

“…However, the presentation of symptoms is often non-specific and generalized to the pelvic region, especially in the context of other pelvic injuries, which can result in delayed presentation and mislead diagnostic prioritization [ 3 , 5 ]. Nonetheless, ultrasonography has been found to maintain the highest degree of diagnostic sensitivity and accuracy when evaluating testicular pathologies, including testicular rupture [ 2 , 4 ].…”

“…Motorcycle collisions specifically are associated with an increased risk of testicular trauma and rupture [1]. Testicular rupture is characterized as disruption of the surrounding tunica albuginea [2][3][4]. However, the presentation of symptoms is often nonspecific and generalized to the pelvic region, especially in the context of other pelvic injuries, which can result in delayed presentation and mislead diagnostic prioritization [3,5].…”
