Background: Cervical ripening is prerequisite of successful induction of labor. Vaginal misoprostol and Foley's catheter placement have been widely used for this purpose but the data are not always sufficient. We attempted to determine which (misoprostol versus Foley's catheter) is more effective/safer in Nigerian setting. Methods: A randomized controlled trial was performed at Federal Teaching Hospital Abakaliki, Nigeria, involving 135 term pregnant women requiring cervical ripening and labor induction. Participants were randomly allocated to misoprostol versus catheter group. The following were recorded/measured/analyzed: Bishop's score, age, parity, body mass index, gestational age, labor duration, indication, oxytocin use, mode of delivery, and Apgar score. Chi square test and t test were used where appropriate. Results: At 24 hours, all of misoprostol group were either in labor, had ripe cervix, or had delivered, whereas 35.4% of catheter group had still unripe cervix (x 2 = 29.856, P = 0.0001). Misoprostol group was less likely to require oxytocin infusion (x 2 = 52.600, P = 0.0001) and less likely to require cesarean delivery (cesarean: misoprostol versus catheter: 11% versus 34% (x 2 = 9.800, P = 0.001)). Total medical cost for misoprostol was less than that of catheter (x 2 = 14.703, P = 0.0001). Conclusion: 50 μg vaginal misoprostol, compared with catheter placement, was more effective, cheaper, and safe as a procedure of cervical ripening before induction of labor.

Mục tiêu: Đánh giá hiệu quả khởi phát chuyển dạ bằng Propess đặt âm đạo ở thai kỳ trưởng thành. Đối tượng và phương pháp nghiên cứu: Nghiên cứu cắt ngang gồm 79 thai kỳ trưởng thành, có chỉ định khởi phát chuyển dạ tại Bệnh viện Đa khoa Quốc tế Vinmec Nha Trang từ tháng 5/2022 đến tháng 8/2023 với đánh giá cổ tử cung bằng thang điểm Bishop. Thai kì trưởng thành có chỉ định khởi phát chuyển dạ khi điểm Bishop ≤ 5 điểm và không có chống chỉ định sinh đường âm đạo. Kết quả: Tuổi thai trung bình ở nhóm thai phụ khởi phát chuyển dạ là 39,7 ± 1,1 tuần với tỉ lệ thai kỳ con so chiếm 77,2%. Chỉ định khởi phát chuyển dạ do thai quá ngày dự sinh chiếm tỷ lệ cao nhất với 38%. Chỉ số Bishop trong 24 giờ đặt thuốc tăng lên so với trước khi đặt thuốc là 6,7 điểm. Tỉ lệ khởi phát chuyển dạ thành công ở thai kỳ trưởng thành có chỉ định khởi phát chuyển dạ là 88,6%. Khởi phát chuyển dạ đến pha tích cực là 86,1%. Tỉ lệ khởi phát chuyển dạ thành công sinh đường âm đạo là 62% tổng mẫu nghiên cứu. Tỉ lệ sản phụ sinh đường âm đạo trong vòng 12 giờ sau đặt Propess là 53,0%. Trong số 30/79 ca mổ lấy thai, chỉ định phẫu thuật vì cổ tử cung không tiến triển chiếm tỉ lệ cao nhất là 53,3%. Đa số các trường hợp khởi phát chuyển dạ bằng Propess đặt âm đạo đều không có tác dụng phụ nghiêm trọng và tai biến. Kết luận: Khởi phát chuyển dạ bằng Propess đặt âm đạo là một phương pháp khởi phát chuyển dạ bằng thuốc an toàn và hiệu quả cao, ít tai biến.
