Background: head injuries (HI) are considered as a major public health concern across the world. This study aims to explore the incidence rate and spatial distribution of HI incidence at rural district levels in Zanjan province, Iran from 2014-2018.Materials & Methods: This study was a cross-sectional and geospatial analysis of head injury incidence pattern in rural areas at Zanjan province, Iran. Data were collected from nine hospital information system databases. Age-adjusted incidence rate and three different spatial analysis methods (Spatial autocorrelation, hotspot analysis and Anselin Local Moran's I) were used to detect the potential high-risk areas of HI incidence in the study area.Results: 4562 patients were registered at Zanjan hospitals due to HI from 2014-2018. The age-adjusted incidence rate of HI was 429 cases (95% CI, 418,443) per 100,000 person which increased from 74 cases in 2014 to 86 cases in 2018, (an 18% increase, P<0001). The highest incidence rate observed among men (80%, P<0001) and at the age group of 15-29 (44.4%, P<0001). Qarabolagh region had the highest incidence rate and five hotspot, seven coldspot, two high-high cluster and seven low-low cluster of HI incidence were detected using spatial analysis.Conclusion: This study provided an overview about the incidence rate and spatial pattern of HI incidence at finer geographical level at the northwest of Iran. This study detected high-risk areas and also showed a significant relationship between HI, geographical areas and genders, which can provide useful information for local health authorities to apply prevention programs for reducing the burden of HI in the society.

Objective: To evaluate the occurrence of head and face injuries and associated factors among victims of automobile accidents in a trauma center. Material and Methods: A cross-sectional study was developed through the analysis of 9,734 medical records of children and adults hospitalized due to external causes in the Regional Hospital of Emergency and Trauma at Campina Grande, Brazil. Data were analyzed with the SPSS software version 15, also using the Pearson's chi-square test. The significance level adopted was 5 %. Results: Cases of automobile accidents accounted for 25.9% (n = 463) of hospitalizations due to external causes, and 83.8 % of victims were male and 16.2 % were female, representing a sex ratio of 5.1:1. As for age group, most had between 20 and 29 years (38.1%) and with respect to the day of the week, 40.8 % of incidents occurred over the weekend. The occurrence of injury was greater on the face (17.4%) than on the head (7.8%) and most victims had a single injury (58.5%). The existence of bone fractures was observed in 76.9 % of victims and 14.7 % had functional impairment. There was association between variables age (p < 0.001), sex (p < 0.001), day of the week (p < 0.001), presence of bone fractures (p < 0.001) and functional impairment (p = 0.002) with the occurrence of automobile accidents. Conclusion: The most automobile accidents occur on weekends and affect young male individuals, with frequent presence of fractures among victims. The occurrence of injury was greater on the face than on the head. Factors such as age, sex, presence of bone fractures and functional impairment was associated with automobile accidents.

Mild head trauma in elderly patients: experience of an emergency department

Introduction We evaluated the risk profile of elderly patients who came to the emergency department for mild head trauma. The primary goal was to determine the difference in the incidence of posttraumatic intracranial hemorrhage (ICH) after minor head injury (MHI). The secondary objective was to assess worse outcome, such as: hospitalization rate, rate of re-admission, need of neurosurgery. We also assess the admission process times and length of hospital stay. The ultimate goal was to optimize the diagnostic-observational management of minor head trauma in elderly patients. Material and methods We evaluated all patients with MHI who came to our emergency department during 2017 and 2018. All patients underwent computed tomography. Results We enrolled 2325 patients, of whom 1094 were 75 years of age or older. The population was divided into two categories according to age: The “elderly population” was 75 or older, and the younger patients were younger than 75. The elderly population, in comparison with the younger patients, had a higher rate of ICH (12.1% versus 5.1%), a higher hospitalization rate (11.7% versus 5.5%), and a higher rate of readmission within 30 days (6.8% versus 3.2%). The elderly population also had longer admission process times (8 h, 25 min, versus 4 h, 09 min) and longer lengths of hospital stay (9 h, 41 min, versus 5 h, 29 min). Of the younger patients, 92% (versus 41% of the elderly population) did not take any drugs, 6% (versus 39%) were receiving antiplatelet therapy, 1% (versus 13%) took vitamin K antagonists, and 1% (versus 7%) took oral direct-acting anticoagulants. Logistic regression models revealed that a 1-year increase in age raised the risk of bleeding by 2% on average; this finding was statistically significant (odds ratio [OR], 1023/year, p < 0.001). The rate of ICH increased significantly after the age of 75, by 180% (OR, 2.82; p < 0.001). Conclusions These data suggest that age is an independent risk factor for ICH, whereby the age of 75 entails a 180% increase in the risk of bleeding.
