Background/Aims: Pancreatic head resection is performed with low mortality, but morbidity remains high. Extensive preparation, long operating times, intraoperative blood loss and the need for blood transfusions are risk factors for postoperative morbidity. The aim of our study was to evaluate the feasibility and safety of the ultrasonic dissection device in pylorus-preserving duodenopancreatectomy (PPPD). Methods: Fifty consecutive patients who underwent PPPD with an ultrasonic dissection device (group 1) were compared with a match-controlled group of 50 consecutive patients who underwent PPPD with conventional dissection techniques (group 2). Duration of surgery, intraoperative blood loss, blood units, complications, mortality and duration of hospital stay were analyzed. Results: There was no difference in age, gender or BMI between groups. In group 1, mean blood loss (446 ± 281.8 ml, p = 0.008) and number of blood units (0.32 ± 0.86, p = 0.001) were significantly lower than in group 2 (819 ± 915.4 ml; 1.36 ± 2.83 units). Duration of surgery was shorter in group 1 (345.6 vs. 373 min, p = 0.28). The rate of postoperative complications, mortality and hospital stay were not significantly different. Conclusions: Use of an ultrasonic dissection device in PPPD might significantly reduce intraoperative blood loss and the need for blood transfusions. These results should be verified in a prospective randomized controlled trial.

Comparison of short-term postoperative complications of thyroidectomy using ligature and suture ligation

Background:Thyroidectomy is one of the most common forms of operations in general surgery. Regarding the status and importance of thyroidectomy and protective measures for avoiding the surgical complications, we aimed at investigating the short-term postoperative complications of thyroidectomy by applying the two methods of ligature use and suture ligation of vessels.Settings:Department of Surgery, Shahid Sadoughi University of Medical Sciences, Yazd, Iran.Materials and Methods:This Double-blind randomized clinical trial study conducted on 140 candidates of thyroidectomy. The patients were randomlyequally allocated assigned to two groups of 70. In the ligature group, thyroidectomy was performed via ligature use, and in the suture ligation group, it was done with suture ligature. Using a specific questionnaire, the required demographic information, type of surgical operation, duration of surgery, rate of bleeding, the preoperative and postoperative serum calcium concentration, hypocalcemia symptoms, hoarseness, and laryngoscopic findings were collected and analyzed.Results:The findings revealed that the average time of total and subtotal thyroidectomy lasted 93.94 ± 10.53 min with ligature use and 134.15 ± 13.57 min with suture ligation. No case of postoperative bleeding was observed in this study. Two patients (17.1%) in the suture ligation group and two patients in the ligature group showed signs of hypocalcemia. Six patients developed hoarseness of whom five belonged to the ligature group and one belonged to the suture ligation group.Conclusions:Regarding the fact that on the whole, duration of thyroidectomy was shorter in ligature approach compared to the suture ligation, it can be considered as a suitable method to substitute suture ligation. However, meticulous precautions should be taken specifically with regard to the efferent laryngeal nerve and hoarseness.

ObjectiveTo characterize adverse events, provide a safety profile, and understand patient outcomes after complications arising from LigaSure and Harmonic use in the head and neck region.MethodsRetrospective analysis of adverse events from the Manufacturer and User Facility Device Experience (MAUDE) between January 2013 and 2023.ResultsA total of 158 LigaSure and 159 Harmonic events were extracted. There were significantly more reports of Harmonic device overheating (6.9% vs. 0.6%, p = 0.003) and spontaneous self‐activation (4.4% vs. 0%, p = 0.032). Although intra‐operative and post‐operative complications were similar among both groups, there were significantly more intra‐operative bleeding events for LigaSure as compared to Harmonic (8.2% vs. 0.0%, p = 0.001).ConclusionUnderstanding technical complications and adverse events attributable to LigaSure and Harmonic devices enables the development of clinically relevant risk mitigation strategies. Surgeons should avoid improper use by remaining vigilant of device functionality and temperature changes.

Thyroidectomy with LigaSure

Abstract: The application of LigaSure using meticulous surgical technique provides a new option for a safe thyroidectomy. Moreover, the duration of the LigaSure procedure is significantly shorter, and there are fewer complications as compared with the classic surgical thyroidectomy technique.
