The aim of the study was to assess the validity and reliability of using the Portapres® to measure toe blood pressure during rest and exercise. Construct validity, concurrent validity, and interday reliability were assessed by measuring toe (Portapres®)) and brachial blood pressure in 16 nondisabled participants on consecutive days. Construct validity was assessed by pedaling on a cycle ergometer (6 revolutions per minute) and comparing the measured toe blood pressure to an estimated value based on orthostatic factors. Concurrent validity was assessed by comparing toe and brachial blood pressure during supine rest and following 10 min of cycling exercise. Interday reliability was assessed by recording toe and brachial blood pressure during supine rest on a second day. Construct validity analysis shows that the toe blood pressure signal was moderately correlated with the changes in heart–toe distance and that changes in toe blood pressure during slow cycling were similar to the estimated value. Resting toe and brachial mean arterial blood pressure showed concurrent validity with only a fixed bias explained by the change in orthostatic pressure and the toe–brachial index. Furthermore, cycling exercise was associated with an increase in brachial and a decrease in toe mean blood pressure. The interday reliability analysis showed no proportional or fixed bias for mean arterial blood pressure. Our study showed the feasibility of using the Portapres® to measure toe blood pressure during movement and can be used to study the effect of movement‐related forces during cycling on toe blood pressure.

Tilt angles and positive response of head-up tilt test in children with orthostatic intolerance

This study aimed at examining three tilt angle-based positive responses and the time to positive response in a head-up tilt test for children with orthostatic intolerance, and the psychological fear experienced at the three angles during head-up tilt test. A total of 174 children, including 76 boys and 98 girls, aged from 4 to 18 years old (mean 11.3±2.8 years old), with unexplained syncope, were randomly divided into three groups, to undergo head-up tilt test at the angles of 60°, 70° and 80°, respectively. The diagnostic rates and times were analysed, and Wong-Baker face pain rating scale was used to access the children's psychological fear. There were no significant differences in diagnostic rates of postural orthostatic tachycardia syndrome and vasovagal syncope at different tilt angles during the head-up tilt test (p>0.05). There was a significant difference, however, in the psychological fear at different tilt angles utilising the Kruskal-Wallis test (χ2=36.398, p<0.01). It was mildest at tilt angle 60° utilising the Kolmogorov-Smirnov test (p<0.01). A positive rank correlation was found between the psychological fear and the degree of tilt angle (r(s)=0.445, p<0.01). Positive response appearance time was 15.1±14.0 minutes at 60° for vasovagal syncope children. There was no significant difference in the time to positive response, at different tilt angles during the head-up tilt test for vasovagal syncope or for postural orthostatic tachycardia syndrome. Hence, it is suggested that a tilt angle of 60° and head-up tilt test time of 45 minutes should be suitable for children with vasovagal syncope.

Impact of body tilt on the central aortic pressure pulse

The present work was undertaken to investigate, in young healthy volunteers, the relationships between the forward propagation times of arterial pressure waves and the timing of reflected waves observable on the aortic pulse, in the course of rapid changes in body position. 20 young healthy subjects, 10 men, and 10 women, were examined on a tilt table at two different tilt angles, −10° (Head-down) and + 45° (Head-up). In each position, carotid-femoral (Tcf) and carotid-tibial forward propagation times (Tct) were measured with the Complior device. In each position also, the central aortic pressure pulse was recorded with radial tonometry, using the SphygmoCor device and a generalized transfer function, so as to evaluate the timing of reflected waves reaching the aorta in systole (onset of systolic reflected wave, sT1r) and diastole (mean transit time of diastolic reflected wave, dMTT). The position shift from Head-up to Head-down caused a massive increase in both Tct (women from 130 ± 10 to 185 ± 18 msec P < 0.001, men from 136 ± 9 to 204 ± 18 msec P < 0.001) and dMTT (women from 364 ± 35 to 499 ± 33 msec P < 0.001, men from 406 ± 22 to 553 ± 21 msec P < 0.001). Mixed model regression showed that the changes in Tct and dMTT observed between Head-up and Head-down were tightly coupled (regression coefficient 2.1, 95% confidence interval 1.9–2.3, P < 0.001). These results strongly suggest that the diastolic waves observed on central aortic pulses reconstructed from radial tonometric correspond at least in part to reflections generated in the lower limbs.
