Toward standardization of carbohydrate-deficient transferrin (CDT) measurements: I. Analyte definition and proposal of a candidate reference method

Abstract: An alcohol-associated change in the serum transferrin glycoform pattern, carbohydrate-deficient transferrin (CDT), is used as a biomarker of chronic moderate to heavy alcohol consumption. A current limitation in CDT analysis is the lack of standardization, which hampers clinical and analytical comparison between studies. This situation prompted initiation of a Working Group (WG) on CDT Standardization under the auspices of the International Federation of Clinical Chemistry and Laboratory Medicine (IFCC). The s… Show more

“…This prompted the International Federation of Clinical Chemistry and Laboratory Medicine (IFCC) to initiate a Working Group aiming for standardization of CDT measurements (WG-CDT). In its first publication [ 7 ], disialotransferrin (disialylated monoglycan transferrin) was defined as the measurand for a reference method and the target analyte for the standardization work, and high-performance liquid chromatography (HPLC) [ 8 ] with photometric detection was proposed as the candidate reference method. A network of reference laboratories running this method was formed demonstrating good intra-and inter-laboratory performance [ 9 ].…”

“…These actions would improve the diagnostic characteristics of CDT as a biomarker for chronic alcohol consumption and promote comparability between laboratories. As a result of these measures, the quality of cooperation between laboratory and court medicine would be improved, along with medical diagnostics and patient care (12,13,25,26).…”

“…For instance, differen tial diagnosis of infectious mononucleosis and serum sickness from the healthy state is based on the reactivity of heterophil antibodies to specific viral carbohydrate antigens, Paul-Bunnell antigen, N-glycolylneuraminic acid and Forsmann antigen (57-59). Well known examples are also glycosylated hemoglobin in monitoring diabetic patients (63), or abnormally glycosylated bands of erythrocyte membranes, as well as carbohydrate-deficient transferrin for diagnosis of inherited or acquired disorders of glycosylation and alcohol abuse (64,65).…”
