Toxic epidermal necrolysis; 15 years’ experience in a Dutch burns centre

Abstract: Because of a potentially fatal outcome, fast referral of a patient suspected of TEN to a specialized centre (mostly a burns unit or specialized dermatology centre) for expert wound management and tailored comprehensive care is strongly advised and contributes to survival.

“…The estimated mortality from SJS is 10%, SJS/TEN overlap 30% and TEN almost 50% [9].Various cohorts on SCAR have been described since the 1990s [79][80][81][82][83][84][85][86][87][88][89][90][91][92][93] from Europe, United States, South Asia and the Asia Pacific (Table 3). Most of these described cohorts included both adult and paediatric inpatients, with only a limited number describing organ and systemic manifestations of SCAR.…”

“…Lefolyása kiszámíthatatlan, igen magas mortalitási arány jellemzô. Elôfordulási gyakorisága a teljes populációt tekintve 0.4-1.8:1000000, enyhe nôi túl-súly figyelhetô meg (1)(2)(3)(4)(5) (3)(4)(5)(6)(7)(8)(9)(10)(11). Patomechanizmusa nem teljesen tisztázott, azonban a gyógyszer, illetve gyógyszermetabolitok által beindított immunológiai reakció eredményeként aktiválódott specifikus cytotoxikus T-sejtek igen fontos szerepet játszanak a hámsejtek károsodásában, emellett a TNF-α, IFN-γ, IL-2 fokozott expressziója révén közvetített, illetve a Fas-FasL interakció révén létrejövô keratinocyta apoptosis is szerepet játszik benne (1, 2).…”

“…A kialakuló hólyagok igen sérülékenyek, a hám felsô része "letö-rölhetô", rövid idô alatt masszív denudáció alakulhat ki (3)(4)(5). Differenciál diagnosztikai szempontból az exfoliatív toxint termelô Staphylococcus aureus által elôidézett "forrázott bôr szindróma", autoimmun hólyagos betegségek, akut graft-versus-host betegség, egyéb súlyos gyógyszer adverz reakciók (generalizált bullosus fix gyógyszerexan-thema, akut generalizált exanthematosus pustulosis, gyógyszer indukálta erythroderma, DRESS-szindróma) jö-hetnek szóba (1,(3)(4)(5).…”
