Transmission of a Klebsiella pneumoniae clone harbouring genes for CTX-M-15-like and SHV-112 enzymes in a neonatal intensive care unit of a Kuwaiti hospital

Abstract: The spread of antibiotic-resistant bacteria has become a large problem in most countries including Kuwait. This antibiotic resistance is usually due to the production of extended-spectrum b-lactamase (ESBL) enzymes such as SHV, TEM and CTX-M. This study reports the emergence and spread of an ESBL-producing Klebsiella pneumoniae clone in a neonatal intensive care unit (NICU) in a Kuwaiti hospital. Eight ESBL-producing K. pneumoniae isolates were from blood cultures of seven neonates, and two were from the finge… Show more

“…The presence of more than one ESBL in a single isolate is being increasingly detected [17,38,42]. In this study, coexistence of bla SHV and bla CTX-M genes was detected in 20.5 % of the strains.…”

“…These findings imply that VEB-type ESBLs are common in the Middle East. The identification of SHV-112 as a novel ESBL variant has been reported only from Kuwait (83,84). The novel PME-1 ESBL, a PER-7 variant, and the rare GES ESBL produced by Acinetobacter and P. aeruginosa from UAE and Saudi Arabia are other examples of the emerging and rare ␤-lactamases found in the Gulf states (23-25, 57, 58) (Fig.…”

“…It is well known that the ESBL type CTX-M-15 is ubiquitous in the Indian subcontinent (167)(168)(169)(170)(171). If inpatients are colonized with ESBL-or carbapenemases-positive bacteria, endogenous infections and/or patientto-patient transmission can occur, which may account for the high prevalence of CTX-M-15 producers isolated from hospitals in Gulf countries (52,54,56,76,84).…”

“…As opposed to methicillin-resistant Staphylococcus aureus and vancomycin-resistant Enterococci, ESBLs-producing strains are another class of primary pathogenic bacteria (17,18). ESBLs mediated by plasmids are common in Enterobacteriaceae, particularly in E. coli and K. pneumoniae, which have been reported to cause serious infections in hospitals, as well as in the community in different parts of the world (19,20).…”
