Triple assessment of sentinel lymph node metastasis in early breast cancer using preoperative CTLG, intraoperative fluorescence navigation and OSNA

Abstract: Use of CTLG and fluorescence navigation made performing SLNB with high accuracy possible in institutions that cannot use the radioisotope method. OSNA provided accurate intraoperative method, allowing for completion of axillary node dissection during surgery and avoidance of second surgical procedure in patients with positive SLNs, thereby reducing patient distress and, finally, saving hospital costs.

“…These substitutes included the usage of indocyaninegreen fluorescent optical intraoperative SLNB, superparamagnetic iron oxide guided SLNB and CTLG . Meanwhile, CTLG is a well‐known localization method for SLN in breast cancer with very acceptable detection rates . The only drawback of this method is lacking accurate localization tool making it a very successful maneuver for mapping and an accepted part of dual localization with blue dye but not a single dependent method.…”

“…Another one studied the possibility of combination of CTLG with fluorescence imaging as localization methods with promising results . A question is raised about the possibility of accurate localization of the SLN detected by CTLG which offers a detection rate up to 100% …”

“…Furthermore, SLN detection is difficult when the SLN is close to the site of RI injection, due to shine-through artifacts [2]. Another method for visualizing SLNs and lymphatics is computed tomography lymphography (CTL), which does not require special equipment and has recently been applied in melanoma, breast, esophageal, and gastric cancer [1521]. However, there are few reports of CTL for SLN mapping in patients with N0 early oral cancer [2, 22, 23].…”
