Troponin levels in patients with stable CAD

Abstract: Úvod: Srdeční troponiny jsou známy jako specifi cké markery myokardiálního poškození. Zvýšení jejich sérových koncentrací nemusí být vždy spojeno s akutní myokardiální ischemií. Díky rostoucí senzitivitě diagnostických kitů stoupá počet pozitivních nálezů i u pacientů bez akutního koronárního syndromu (AKS). Cíle studie: 1. Stanovit koncentraci vysoce specifi ckého troponinu T (hsTnT) u stabilních nemocných (bez AKS) před elektivním koronarografi ckým vyšetřením (elektivní selektivní koronarografi e [SKG]). 2.… Show more

“…27,28 ). Pre-procedural troponin levels likely mirror the extent of coronary involvement as suggested by studies searching for any association between baseline troponin levels and the SYNTAX score 29,30 . However, no statistically significant association between post-procedural troponin elevation and the SYNTAX score was found in our study.…”

“…serum assay was introduced [11]. There are three types of cTn (C, I, and T), and each type plays different roles in a contractile regulatory complex as follows [12,13]: (1) cTn T binds to the actin filament, (2) cTn C acts as calcium ion binding site, and (3) cTn I inhibits interaction of actin with the myosin heads when the intracellular calcium concentration is insufficient to initiate muscle contraction. CTn exists also in skeletal muscle fibers, but a difference in version of cTn provides a tissue specificity for cardiac muscle [14].…”

“…CTn exists also in skeletal muscle fibers, but a difference in version of cTn provides a tissue specificity for cardiac muscle [14]. Over the other cardiac markers, cTn has advantages with improved performance and superior sensitivity [12,13]. The both types of cTn I and T can be used for detecting myocardial damage, and the accuracies were similar in a direct comparison between the recently available assays [15].…”

“…The risk of myocardial ischemia or infarction is higher in patients with ESRD compared to the general population (4). Cardiac troponins (cTn) can be used to diagnose CAD (5). However, the troponin level is thought to be increased in ESRD patients without coronary events (6).…”
