Twelve cases of hydronephrosis in utero diagnosed by ultrasonography.

Abstract: Twelve examples of hydronephrosis in utero, seven of which were bilateral, were seen at the Johns Hopkins Hospital. Unexpected findings on sonographic examination were: 1) two cases of transient hydronephrosis of moderate severity, persisting over several examinations, which disappeared spontaneously; 2) only two of six examples of ureteropelvic junction obstruction were detected by palpation by competent neonatologists at the time of delivery; 3) polyhydramnios in association with bilateral ureteropelvic junc… Show more

“…This latter finding is in agreement with other authors' experiences which report diagnosis of the disease at a later stage of gestation than the one observed when other uropathies are diagnosed [11,19]. The hypothesis of a late occurrence in gestation seems quite consistent and possibly related to the marked increase in the fetal urinary production.…”

“…Until now eleven papers [1,5,8,10,11,12,14,15,19,20,21] reported a total of 36 cases of fetal hydronephrosis presumably not due to low urinary tract obstruction with controlled neonatal outcomes. Therapeutic abortions were performed in two cases, postnatal surgery in twenty, severe associated malformations occurred in five cases and neonatal mortality was 14% mostly due to malformations other than hydronephrosis.…”

“…Vaginal delivery should be possible, unless the presence of additional pregnancy-related problems dictates other-wise. Harrison et al (1981) and Sanders and Graham (1982) already previously published reports which document the fact that not all cases of bilateral hydronephrosis detected in utero require invasive intervention. This was also recognized at the Kroc conference on fetal surgery in 1982, which led to recommendations concerning management of obstructive uropathy (Harrison et al, 1982b).…”

“…Analysis of the literature [8,16,26,30,44,57,58,70,78,80,86,103,105,110,113] shows that no large series of hydronephrosis with prenatal diagnosis has been published to our knowledge. Most of the cases described in these reports are part of a general review of [57] reported 11 cases of hydronephrosis, although their study was mainly focused on the problems related to shunting of the urinary tract in utero.…”
