Variability in stem taper surface topography affects the degree of corrosion and fretting in total hip arthroplasty

Abstract: Degradation at the modular head-neck interface in total hip arthroplasty (THA) is predominately expressed in the form of corrosion and fretting, potentially causing peri-prosthetic failure by adverse reactions to metal debris. This retrieval study aimed to quantify variations in stem taper surface topographies and to assess the influence on the formation of corrosion and/or fretting in titanium alloy stem tapers combined with metal and ceramic heads. Four hip stem designs (Alloclassic, CLS, Bicontact and SL-Pl… Show more

“…Despite these concerns, the Goldberg standard is one of the most frequently used approaches to evaluate THA implants worldwide, mainly for retrievals. [48][49][50] Furthermore, we quantified the area damaged by fretting and corrosion to obtain more precise data on the area affected.…”

“…In addition, the scoring method is limited by the experience of the investigator at identifying the four scoring levels. Despite these concerns, the Goldberg standard is one of the most frequently used approaches to evaluate THA implants worldwide, mainly for retrievals 48–50 . Furthermore, we quantified the area damaged by fretting and corrosion to obtain more precise data on the area affected.…”

“…These may subsequently reduce the probability of fluid ingress into the crevice-like gap of the junction due to the toggling effects, thus reducing the susceptibility to corrosion. The presence of such microgrooves is believed to partly compensate for the influence of unavoidable taper angle mismatch which results in a smaller contact area and possible drop in integrity [ 26 , 27 , 28 ]. Although modern junctions are mostly designed and manufactured with microgrooves, research confirms that there is a limited understanding of how these microgrooves affect the junction performance, as evident in the inconsistent and often contradictory results reported by researchers.…”

“…Although modern junctions are mostly designed and manufactured with microgrooves, research confirms that there is a limited understanding of how these microgrooves affect the junction performance, as evident in the inconsistent and often contradictory results reported by researchers. Some research studies confirm no significant role played by these microgrooves in the interface damage [ 29 , 30 ] but some others report higher/lower interfacial damage for these junctions [ 26 , 27 , 28 ]. There is also disagreement on the influence of these microgrooves on the pull/twist-off strength [ 24 , 28 ].…”
