What exactly is catatonia in children and adolescents

Abstract: W niniejszej pracy przedstawiono przegląd literatury dotyczącej zespołu katatonicznego ze szczególnym uwzględnieniem specyfiki wieku rozwojowego. Najnowsze klasyfikacje wprowadziły zmiany w dotychczasowym rozumieniu katatonii. Show more

“…Pediatric catatonia also has a high comorbidity with autism spectrum disorders, reportedly as high as 12-17% in adolescents and young adults with autism (Dhossche & Wachtel, 2010). Catatonic symptoms are similarly common in the autoimmune disease, anti-NMDA receptor encephalitis (Remberk et al, 2020). Utilizing medical workup and appropriate symptom characterization tools such as the BFCRS or the less-widely utilized pediatric catatonia rating scale (PCRS) is thus valuable to accurately identify and appropriately treat this complex and underdiagnosed syndrome.…”

“…Three subtypes are recognized to include hyperkinetic (excited), hypokinetic (stuporous), and malignant with cases ranging from socially debilitating to life threatening. A patient’s manifesting impediments in physical activity and reduced contact with their external environment may lead to marked disability, with pediatric catatonia increasing risk of premature death by 60-fold in interval follow-up (Remberk et al, 2020; Edinoff et al, 2021).…”

“…Pediatric catatonia is believed to be a rare condition ( 7 , 8 ), but challenges in recognition and variability in the presentation may lead to under-diagnosis ( 9 ). Catatonia should be treated quickly because failing to prioritize this problem can result in a suboptimal outcome if the patient is started on benzodiazepines or ECT too late ( 10 ).…”

“…The diagnostic criteria for catatonia in the current DSM-5 require three or more of the following symptoms: stupor, waxy flexibility, catalepsy, mutism, posturing, negativism, stereotypes, mannerisms, grimacing, agitation, echopraxia, and echolalia [ 8 ]. These diagnostic criteria apply to both adults and children, but in children, catatonia often presents as a result of somatic conditions or substance use.…”

“…These diagnostic criteria apply to both adults and children, but in children, catatonia often presents as a result of somatic conditions or substance use. Catatonia associated with schizophrenia in children is considered serious and needs immediate attention, as there is a greater opportunity for poor prognosis [ 8 ]. Patients who develop catatonia accompanied with acute autonomic instability have an increased risk of complications and mortality [ 9 , 10 ].…”
