What is the contribution of embryo-endometrial asynchrony to implantation failure?

Abstract: Embryo-endometrial synchrony is critical for successful implantation. There is an unmet need for improved precision in the evaluation of endometrial development to permit better synchronization of the embryo and the endometrium prior to implantation.

“…Implantation is a major limiting step in IVF procedures, and it is incredibly frustrating for infertility specialists that almost 20% of patients have RIF . Because there is no solid evidence on how to manage RIF, treatments are scarce and those used are primarily empirical.…”

“…Despite advances in assisted reproduction technologies (ART), the overall pregnancy rate (PR) of in vitro fertilization (IVF) is only about 40%–50%, and about 20% of patients experience repeated implantation failure (RIF) . Thus, improving IVF outcomes, especially for patients with RIF, is an important research topic.…”

“…Recently, a clinical pregnancy rate of 19.4% (14/72 ET cycles) and live birth rate of 15.2% (11/72 ET cycles) with a multiple conception rate of 28.5% (4/14 pregnancies) in 81 women with HH were reported . Ovarian stimulation associated with supraphysiological levels of steroid hormones can disturb optimal uterine receptivity and embryo implantation by perturbation of embryo‐endometrial synchrony . It is possible that the endometrial condition was not optimal for implantation in women with HH after ovarian stimulation with exogenous gonadotropins, and ET with multiple fresh embryos demonstrated relatively low OHSS and miscarriage rates but a high multiple pregnancy rate .…”

“…Implantation occurs when a free‐floating mature blastocyst attaches to the endometrium, invades the stroma and establishes the placenta. For this process to be successful, the endometrium must be in a receptive state (Teh, Mcbain, & Rogers, ), what was the result of the synchronized and integrated interaction among multiple factors and genes (Cha, Vilella, Dey, & Simon, ; Zhang et al, ).…”
