BackgroundThe aims of this retrospective study were to evaluate the maternal and prenatal outcomes between 35 years and older pregnancies and younger pregnancies, and the effects of the age of pregnancy, mother and newborn.MethodsPregnant women who gave birth in Vakif Gureba Training and Research Hospital, Clinic of Obstetrics and Gynecology in 2006 were retrospectively screened. Pregnant women aged 35 years and over were included in this study and the pregnant women between age range of 30 - 34 years were included in the control group.ResultsPregnancy rate was found as 7.1% in 35 years and older women in all the deliveries, cesarean delivery rate was found as 46.1% in this group at 1 year period. However, cesarean delivery rate was 40.9% in the control group. Cesarean delivery rate was found as 31.6% in all the deliveries. The most common cause of cesarean section indication was fetal distress in advanced maternal age (AMA) (11.7%), whereas previous cesarean section was found as the most common cause in the control group (15.1%).ConclusionNo significant difference was found between AMA group and normal pregnancies in terms of preterm labor, caesarian section, morbidity, mortality and chronic diseases such as hypertension and diabetes mellitus.
The present study was conducted to identify endometrial and myometrial lesions coexisting with adenomyosis, and to evaluate the clinicopathological characteristics of endometrial adenocarcinomas associated with adenomyosis. A retrospective analysis of the resected uterine specimens of 319 patients with adenomyosis admitted between January 1, 2014 and August 1, 2017 was performed. The endometrial and myometrial lesions coexisting with adenomyosis were evaluated. The clinicopathological prognostic factors, including tumor grade, myometrial invasion, lymphovascular space involvement, lymph node invasion, pathological stage and recurrence, were analysed. For data analysis, the Chi-squared test was used and a P-value of <0.05 was considered to indicate statistically significant differences. The mean age of the patients was 52.1 years. A total of 32 patients had endometrial carcinoma associated with adenomyosis. In addition to endometrioid adenocarcinoma of different grades, rare clear cell carcinoma cases were also observed. Two cases of malignant mesenchymal tumors (one low-grade endometrial stromal sarcoma and one leiomyosarcoma) were also diagnosed. Therefore, patients presenting with abnormal uterine bleeding should undergo thorough evaluation for the presence of adenomyosis and/or leiomyoma(s). Although the cases of endometrial adenocarcinoma associated with adenomyosis generally had a good prognostic outcome, there were also rare cases of patients with agressive tumor morphology. The inflammatory and tissue response arising around the foci of adenomyosis generate a preventive mechanism against the invasion of adenocarcinomas coexisting with adenomyosis. This response is likely the primary mechanism underlying the good clinical course of these tumors. Therefore, the presence of adenomyosis may be an important factor for the determination of prognosis.
An extremely rare case of a “primary endometrial adenocarcinoma with signet-ring cells” is presented in this study with microscopical images of the characteristic coexistence of the tumour and intermediate precancerous areas containing signet-ring cells.
BACKGROUND/AIMSThe study aimed to explore the perceptions of hospitalized COVID-19 patients' experiences regarding the disease and treatment process.
MATERIAL and METHODSThe study was carried out as qualitative research at a hospital in Turkey between June 17, 2020 and July 7, 2020. The sample consisted of eight COVID-19 patients hospitalized in service after intensive care treatment. The data were collected through an in-depth individual interview form. Each interview was transcribed verbatim, and a thematic analysis was performed. The Standards for Reporting Qualitative Research checklist was followed.
RESULTSThree main themes were identified for patients' perceptions of the disease experiences and treatment process. The patients experienced negative emotions after being diagnosed with COVID-19, and they spent the isolation and treatment process communicating with their beloved ones over the phone, watching television, and praying. Having an infectious disease caused them to have anxiety and sadness, and they reported satisfaction with the physicians and nurses during the treatment process.
CONCLUSIONPsychological and sociocultural factors, religious values, and health policies can be effective in patients' perception of disease and treatment. It is recommended for healthcare professionals to be aware of the psychosocial problems of the hospitalized COVID-19 patients during the disease and treatment process, monitor them for post-traumatic stress disorder, and provide the necessary support.
