Background/Aims: Hypokinetic dysarthria in Parkinson disease (PD) hinders the ability to verbally communicate and interferes with activities of daily living. SPEAK OUT!® is a therapy program designed to improve functional communicative ability. In contrast to the Lee Silverman Voice Treatment program, SPEAK OUT!® promotes speaking with intent to effect loud speech. This study evaluated the efficacy of SPEAK OUT!® in persons with idiopathic PD in 3 domains: self-reported voice handicap, clinical ratings of dysarthria and prosody, and acoustic analysis of prosody. Participants and Methods: Pre-/post-therapy data included PD participants’ scores on the Voice Handicap Index (VHI) and the Voice-Related Quality of Life (V-RQOL) questionnaire, audio recordings, perceptual evaluation scores, and demographic data, such as age, sex, handedness, diagnosis, and onset of PD. Results: Participants achieved a statistically and clinically significant improvement in speech intensity, pitch range, normalized pairwise variability index for pitch, sustained vowel duration, reading intelligibility, and vocal quality after SPEAK OUT!® training, consistent with both of the self-report voice scores, i.e., the VHI and the V-RQOL, and with the perceptual speech evaluation scores. Longer PD duration was associated with lowered efficacy. Conclusions: SPEAK OUT!® is effective and should be administered as early as possible after disease onset.
This study aimed to determine the relationships between particulate matter risk perception, knowledge, and perceived barriers and health-promoting behaviors among college students. Methods: Data for this cross-sectional study were collected from September 1 to 30, 2017. The study sample consisted of 85 students from a university, Seoul. Students not living in the Seoul metropolitan area during the spring 2017 semester were excluded from participation. Pearson' s correlation coefficient was used to identify relationships among study variables. Results: A significant positive correlation existed between particulate matter risk perception and health-promoting behaviors related to particulate matter (r= .51, p< .001). Among the risk perception subdomains, attention (r= .47, p< .001) and health effect (r= .55, p< .001) showed strong positive relationships with health-promoting behaviors. No significant relationships were found between knowledge (r= .12, p= .288) or perceived barriers (r=-.12, p= .264) and health-promoting behaviors related to particulate matter. Conclusion: Based on the study results, strategies for enhancing particulate matter risk perception are needed to increase the level of health-promoting behaviors related to particulate matter among college students.
The term COVID-19 “long haul” originated on social media and was later studied by the scientific community. This study describes content related to persistent COVID-19 symptoms on YouTube. The 100 most viewed English-language videos identified with the keywords “COVID-19 long haul” were assessed for video origin, engagement, and content related to COVID-19 long-haul. The findings indicate that the majority of videos were uploaded by television or internet news (56%), followed by consumers (members of the public, 32%), health professionals (only 9%), and lastly by entertainment TV (non-news programs, 3%). Videos originating from entertainment TV were significantly more likely to be “liked” than videos from other sources. The most commonly mentioned long-haul symptoms in the videos were physical (fatigue, 73%; difficulty breathing/shortness of breath, 56%; and joint or muscle pain, 49%) and cognitive (difficulty thinking or concentrating; 69%). The case of COVID-19 long haul demonstrates that social media are significant fora whereon the public identify health concerns. It is necessary for healthcare professionals to assume an active and responsible role in social media.
Articles from open access and local journals are important resources for research in Korea and the usage trends of these articles are important indicators for the assessment of the current research practice. We analyzed an institutional collection of published papers from 1998 to 2014 authored by researchers from Seoul National University, and their references from papers published between 1998 and 2011. The published papers were collected from Web of Science or Scopus and were analyzed according to the proportion of articles from open access journals. Their cited references from published papers in Web of Science were analyzed according to the proportion of local (South Korean) or open access journals. The proportion of open access papers was relatively stable until 2006 (2.5 ~ 5.2% in Web of Science and 2.7 ~ 4.2% in Scopus), but then increased to 15.9% (Web of Science) or 18.5% (Scopus) in 2014. We analyzed 2,750,485 cited references from 52,295 published papers. We found that the overall proportion of cited articles from local journals was 1.8% and that for open access journals was 3.0%. Citations of open access articles have increased since 2006 to 4.1% in 2011, although the increase in open access article citations was less than for open access publications. The proportion of citations from local journals was even lower. We think that the publishing / citing mismatch is a term to describe this difference, which is an issue at Seoul National University, where the number of published papers at open access or local journals is increasing but the number of citations is not. The cause of this discrepancy is multi-factorial but the governmental / institutional policies, social / cultural issues and authors' citing behaviors will explain the mismatch. However, additional measures are also necessary, such as the development of an institutional citation database and improved search capabilities with respect to local and open access documents.
Introduction: SPEAK-OUT!® is a behavioral treatment for hypokinetic dysarthria in persons with Parkinson’s Disease (PD) that has become an alternative to the gold-standard Lee Silverman Voice Treatment (LSVT) in recent years. Acoustic evaluation of the efficacy of SPEAK-OUT!® therapy has focused on prosody. The purpose of this study was to investigate SPEAK-OUT!® efficacy in terms of vocal quality and its impact on quality of life. Vocal quality was measured acoustically using Cepstral Peak Prominence (CPP) analysis and the Acoustic Voice Quality Index (AVQI) and perceptually using clinical ratings of speech performance. Impact on quality of life was measured with the Voice Handicap Index (VHI) and Voice-Related Quality of Life (V-RQOL). An additional goal of this study was to investigate whether dysarthria severity and disease duration were predictive of changes in voice quality due to SPEAK-OUT!®. Methods: Pre- and post-therapy data included PD participants’ acoustic and perceptual ratings of audio recordings. Demographic data included age, sex, diagnosis, duration of PD and severity of dysarthria. Results: Participants achieved significant improvement in vowel and sentence CPPS mean score as well as in their AVQI score post SPEAK-OUT!® treatment. Improvements in AVQI correlated negatively with disease duration and positively with dysarthria severity Discussion/Conclusion: SPEAK-OUT!® is effective in improving voice quality in patients with hypokinetic dysarthria due to idiopathic PD. Patients with more severe dysarthria and with a shorter disease duration may benefit the most, supporting earlier intervention. As for the type of measurement, AVQI combines acoustics from both vowel and sentence contexts, and may therefore be the measure of choice over CPPS (vowel) or CPPS (sentence).
