The special pharmacological properties of adapalene may explain why, when compared with tretinoin, it has an improved therapeutic ratio due to its better tolerance. 10 In our case, and being a localized form, we chose topical adapalene with the aim of achieving a good clinical response as well as minimizing side effects. The lesions were successfully treated with a 2-month course of 0.1% topical adapalene gel in occlusion applied at night only. At the end of this treatment, we observed clearance of crusts and papules with no significant irritation.In conclusion, adapalene is an option when considering an alternative treatment in localized forms of DD. It modules keratinocyte differentiation and has a more specific molecular pharmacological profile than tretinoin, resulting in high efficacy and less irritation.
